Testicular Ischemia and Suspected Torsion(the Diagnostic Value of TWIST)Score Combined with Color Doppler Ultrasound in Pediatric Scrotal Emergency
Objective:To explore the diagnostic value of TWIST score combined with color Doppler ultrasound in pediatric scrotal emergencies.Methods:In 74 cases of children with scrotal redness or discomfort who were admitted to our hospital from January 2018 to January 2023.TWIST score was taken for all children,children with a total score of 3 to 4 were examined by color Doppler ultrasound,the characteristics of pure color Doppler ultrasound images were analyzed,compared with the pathological diagnosis of clinical treatment and surgical results,and the diagnostic value of TWIST score combined with color Doppler ultrasound for pediatric scrotal emergency was analyzed.Results:The TWIST score of 74 children with scrotal urgency was 1-7 with an average score(3.46±1.32)points,28 cases of low-risk infants≤2 points,41 cases of moderate-risk infants 3-4 points,and 5 cases of high-risk infants≥5 points.The pathological findings showed that 6 patients had testicular torsion,67 epididymitis and 1 orchitis among 74 children with scrotal emergency.Color Doppler ultrasound found that 6 children had testicular torsion,60 orchitis and 6 epididymitis;The accuracy of ultrasound diagnosis:91.89%was lower than that of pathological diagnosis:100.00%(P<0.05);The sensitivity and specificity of TWIST score combined with ultrasound for diagnosis of pediatric scrotal emergency were 83.23%and 94.14%respectively,which were significantly higher than that of single diagnosis(P<0.05).Conclusion:The combination of TWIST score and color Doppler ultrasound has high diagnostic value,high diagnostic sensitivity and specificity for pediatric scrotal emergencies.
