公立医院高值医用耗材综合价值评价体系构建与实践探索——以硬脑膜补片为例

目的:以硬脑膜补片为例探讨高值医用耗材综合价值评估方法,为医院管理者提供决策支持,提高医院高值耗材精细化管理水平.方法:以2021年7月~2022年3月浙江省某三甲医院硬脑膜补片临床使用数据为基础,结合卫生技术评估标准,建立高值耗材评价体系.依据评价体系中的相关指标参数,对硬脑膜补片成本进行逐一分析,实现对医院采购目录内不同品牌硬脑膜补片的综合价值进行评价.结果:综合价值评分由高到低依次为硬膜修补材料ID、可吸收硬脑(脊)膜补片RDS、生物型硬脑(脊)膜补片NII、生物膜SN.结论:基于卫生技术评估的高值耗材评价体系侧重于耗材自身价值评估,可为医疗机构高值耗材使用管理提供支持和依据,受纳入研究样本量和质量的限制,上述结论尚需更多高质量研究予以验证.
Construction and Practical Exploration of the Comprehensive Value Evaluation System of High-Value Medical Consumables in Public Hospital——Take Dural Mesh as an Example
Objective:To discuss the comprehensive value evaluation method of high value medical consumables,to provide decision support for hospital managers and improve the fine management level of high value consumables.Methods:Based on the clinical use data of dural mesh in a Grade A Hospital in Zhejiang Province from July 2021 to March 2022,the evaluation system of high-value consumables was established.According to the relevant index parameters in the evaluation system,the cost of dural mesh is analyzed one by one to realize the realization of evaluation of the comprehensive value of different brands of dural mesh in the hospital procurement catalogue.Results:The comprehensive value score from high to low was dura repair material ID,absorbable durobon(spinal)membrane patch RDS,biodurobon(spinal)membrane mesh NII,and biofilm SN.Conclusion:The evaluation of high-value consumables system based on health technology evaluation focuses on the value evaluation of consumables themselves,which can provide support and basis for the use and management of high-value consumables in medical institutions.Due to the sample size and quality of the included studies,the above conclusions need to be verified by more high-quality studies.
