目的:探讨早期异位妊娠应用宫腔三维成像、三维高分辨率血流技术(three-dimentional High resolution blood flow image,3D-HD-FLOW)联合断层超声显像技术(tomographic ultrasound imaging,TUI)诊断的临床价值.方法:选取2023年1月～2024年6月本院接诊的疑似早期异位妊娠患者46例,全部患者均接受宫腔三维成像检查、3D-HD-FLOW检查以及TUI检查,以腹腔镜或宫腔镜检查结果作为金标准,对比单一及联合方法的诊断价值.结果:46例疑似早期异位妊娠患者中,经腹腔镜或宫腔镜检查明确诊断41例.宫腔三维成像诊断异位妊娠的灵敏度90.24％、特异度80.00％、准确度89.13％;3D-HD-FLOW诊断的灵敏度85.37％、特异度60.00％、准确度82.61％;TUI的诊断灵敏度92.68％、特异度80.00％、准确度91.30％;联合诊断的灵敏度97.56％、特异度80.00％、准确度95.65％;对比发现,联合检查对不同类型异位妊娠检出率用x2检验后发现P>0.05,没有显著的统计学意义.结论:宫腔三维成像检查、3D-HD-FLOW检查、TUI检查在诊断异位妊娠中具有一定的应用价值,且单一检查及联合检查的诊断效能相近,从经济学上考虑,任何一种单一检查手段都可以选择,缩短检查时间.
Three-Dimensional Imaging of Uterine Cavity Combined with 3D HD-FLOW and TUI in Early Ectopic Pregnancy Application Value
Objective:To explore the clinical value of three-dimensional high resolution flow image ( 3D-HD-FLOW ) combined with tomographic ultrasound imaging ( TUI ) in the diagnosis of early ectopic pregnancy. Methods:A total of 46 patients with suspected early ectopic pregnancy admitted to our hospital from January 2023 to June 2024 were selected. All patients underwent three-dimensional uterine imaging,3D-HD-FLOW examination and TUI examination. Laparoscopic or hysteroscopic examination results were used as the gold standard to compare the diagnostic value of single and combined methods. Results:Among 46 patients with suspected early ectopic pregnancy,41 cases were diagnosed by laparoscopy or hysteroscopy. The sensitivity,specificity and accuracy of three-dimensional imaging in the diagnosis of ectopic pregnancy were 90.24％,80.00％ and 89.13％,respectively. The sensitivity,specificity and accuracy of 3D-HD-FLOW were 85.37％,60.00％ and 82.61％,respectively. The sensitivity,specificity and accuracy of TUI were 92.68％,80.00％ and 91.30％,respectively. The sensitivity,specificity and accuracy of combined diagnosis were 97.56％,80.00％ and 95.65％,respectively. By comparison,it was found that the detection rate of different types of ectopic pregnancy by combined examination was P>0.05 after using the chi square test,and there was no significant statistical significance. Conclusion:Three-dimensional imaging examination of the uterine cavity,3D-HD-FLOW examination,and TUI examination have certain application value in the diagnosis of ectopic pregnancy,and the diagnostic efficiency of single examination and combined examination is similar. From an economic perspective,any single examination method can be chosen to shorten the examination time.
