Application of the MEUS Technique in Preoperative Localization of Perforator Flap
Objective:To investigate the value of MEUS in preoperative localization of perforator flap. Methods:33 patients who underwent flap transplantation were selected,and the color Doppler flow imaging (CDFI) and MEUS were used to detect the number of small vessels in the flap supply area,and compare the difference between perforator vessels and small vessels in the fat layer. The optimal perforator was selected according to the inner diameter,walking,peak flow velocity and resistance index of the perforating vessel. Among the 33 patients,35 flaps were designed with a size range of 3.5cm×5.5cm-15.0cm×11.0cm,and 35 flaps were cut,and each flap contained 1 to 3 perforator vessels. The diagnostic efficacy of CDFI and MEUS was compared using intraoperative outcome as the gold standard. Results:MEUS was better than CDFI in detecting perforator vessels (P<0.05);MEUS was better than CDFI in detecting small blood vessels in the fat layer (P<0.05);Kappa value (0.867) of MEUS in diagnosis of dominant perforators was higher than CDFI (0.715),the sensitivity of MEUS was better than that of CDFI (P<0.05). Conclusion:MEUS is better than CDFI in detecting perforator vessels and blood flow perfusion of fat layer,MEUS is helpful to display microcirculation of flap.
