腹腔镜阑尾切除术的学习曲线研究

目的评估腹腔镜阑尾切除术不同阶段的手术效果,探讨初接触腹腔镜手术的外科医师缩短手术曲线的要点。方法回顾性分析2011年11月-2012年11月由同一组医生连续施行30例腹腔镜阑尾切除术患者的临床资料,按手术先后顺序分为3组(A、B、C组),每组10例。比较各组手术时间、术中出血量、并发症以及住院天数,分析不同阶段的手术效果。结果 A组手术时间、术中出血量、术后住院天数高于B、C组;B、C组间差异无统计学意义。结论初接触腹腔镜手术的外科医生,腹腔镜阑尾切除术是一种比较基础的训练,学习曲线大约为10例。
Study on learning curve of laparoscopic appendectomy
Objective To evaluate the surgical outcomes of laparoscopic appendectomy and to investigate the key points for the beginner surgeons of laparoscopic appendectomy to shorten the learning curve quickly. Methods Clinical data of 30 cases which received laparoscopic appendectomy performed by the same group of the surgeons without any laparoscopic operations were reviewed. The cases were divided into 3 groups(group A, B and C) according to the sequence of the operation. The operation time, blood loss, complications and hospital stay were compared among the 3 groups. Results The operating time, blood loss in group A were significant longer or higher than those in group B and C. while no significant differences were found between the group B and C. Conclusions For the beginner, laparoscopic appendectomy is a good training way. The learning curve is a approximately 10 cases.
