目的探讨高危型人乳头状瘤病毒(HPV)负荷量与细胞学结果为不典型鳞状上皮增生不除外高度上皮内病变(Atypical squamous cel s not except high lesion，ASC-H)的病例中宫颈上皮内瘤变(cervical intraepithelial neoplasia，CIN)的病变程度的相关性。方法对2009年1月-2012年11月就诊于我院妇产科门诊，并经宫颈液基细胞学(Thinprep cytology test，TCT)筛查为ASC-H的127例患者，采用杂交捕获二代(HC-2)试验技术进行HPV-DNA病毒负荷量的检测。检测结果根据病毒负荷量多少分为三组：低病毒负荷量组(1.00-10.00)、中病毒负荷量组(10.00-100.00)和高病毒负荷量组(≥100)，分析HPV病毒负荷量与CIN的病变程度的相关性。结果127例ASC-H患者中，HPV阳性患者99例，其中高病毒负荷量组62例，占62.63%，中病毒负荷量组30例，占30.30%，低病毒负荷量组7例，占7.07%。低病毒负荷量中高程度CIN(HSIL)的发生率为57.14%，中病毒负荷量中高程度CIN(LSIL)的发生率为66.67%，高病毒负荷量中高程度CIN的发生率为87.10%，P =0.032，分布差异显著(P<0.05)。结论基本可以判断在ASC-H病例中，CIN病变程度与HPV病毒负荷量呈正相关，即HPV病毒负荷量越高，CIN病变程度越高。
Discuss on the correlation between the HPV and the degree of Atypical squamous cells not except high lesion
Objective To explore the correlation between the load of high-risk HPV and the degree of cervical intraepithelial neoplasia, which is based on cytology results of patients who have ASC-H. Methods Analyzing the patients who have taken thinprep cytology test in our Hospital and was diagnosed as atypical squamous cells not except high lesion during January 2009 to November 2012. All the patients used the hybrid capture Ⅱ technology to gain the load capacity of HPV. Patients are divided into three groups: the first group is the high HPV load group, which collect all patients whose HPV load is more than 100; the second group is the middle HPV load group, patients in which have HPV load between 10 and 100 the third group is the low HPV load group, which occupies patients who have HPV load between 1 and 10. Results The percent of patients who have senior CIN in high HPV load group, middle HPV load group and low HPV load group is 87.10%, 66.67%, 57.14%, respectively. Conclusion Basically, our research has found a positive correlation between the degree of CIN and the HPV load in patients who have ASC-H.
