Clinical characteristics in conventional MRI-positive and MRI-negative enterovirus 71 encephalitis
Objective Clinical characteristics of conventional MRI-positive as compared to MRI-negative enterovirus 71(EV71) encephalitis are not yet known. Methods We studied 21 conventional MRI-negative and 11 MRI-positive patients with enterovirus 71 encephalitis. All patients underwent conventional MRI(including T1WI, T2WI, and FIAIR) cranial scans and were placed into one of two groups according to MRI results, a MRI-positive group or a MRI-negative group. Clinical characteristics were compared between the two groups. Results Clinical characteristics of the MRI-negative group including age, gender, high fever, rash, limb tremor, headache, vomiting, irritability, tachycardia, tachypnea, hypertesion, disturbance of consciousness, limb weakness, eye movement abnormalities, pneumorrhagia, blood glucose, white blood cell count, CSF pressure, CSF cell count, CSF total protein were not significantly different with the MRI-positive group. Conclusions The positive result from conventional MRI scan may not indicate more severity of clinical feature in EV71 encephalitis patients. To identify the severe cases in EV71 encephalitis patients, enhanced MRI scan or diffusion weighted MRI should be utilize.
