腹腔镜手术联合促性腺激素释放激素类似物治疗卵巢巧克力囊肿的效果
Effect of Laparoscopic Surgery combined with Gonadotropin-releasing Hormone Analogues in the Treatment of Chocolate Cyst of Ovary

摘要
目的 探讨腹腔镜手术联合促性腺激素释放激素类似物治疗卵巢巧克力囊肿的效果.方法 选取 2019 年 6 月至 2022年6月南通市妇幼保健院收治的68例卵巢巧克力囊肿患者,随机分为对照组(n=34,给予腹腔镜手术治疗)与观察组(n=34,给予腹腔镜手术+促性腺激素释放激素类似物治疗),比较两组的治疗效果.结果 观察组治疗有效率为97.06%,高于对照组的76.47%,差异有统计学意义(P＜0.05).治疗后观察组雌二醇(E2)、卵泡刺激素(FSH)、孕酮(P)及黄体生成素(LH)水平分别是(44.26±11.48)pmol/L、(6.74±1.49)U/L、(4.54±0.93)pmol/L、(7.33±1.91)IU/L均明显优于对照组的(67.83±14.57)pmol/L、(18.15±3.72)U/L、(3.26±0.54)pmol/L、(11.15±1.82)IU/L,差异有统计学意义(P＜0.05).治疗后观察组窦卵泡数(8.93±1.24)个多于对照组(7.36±1.18)个,卵巢体积(8.02±1.23)cm3小于对照组的(9.55±1.37)cm3,差异有统计学意义(P＜0.05).随访 1 年,两组月经时间、月经周期比较差异无统计学意义(P＞0.05);观察组妊娠率 67.65%明显高于对照组的38.24%,复发率5.88%低于对照组的17.65%,差异有统计学意义(P＜0.05).结论 在卵巢巧克力囊肿的治疗中,腹腔镜手术联合促性腺激素释放激素类似物的治疗效果显著,可以有效改善患者的卵巢功能,提高妊娠率,降低复发率.
Abstract
Objective To explore the effect of laparoscopic surgery combined with gonadotropin-releasing hormone analogues in the treatment of chocolate cyst of ovary.Methods A total of 68 patients with chocolate cyst of ovary admitted to Nantong Maternal and Child Health Hospital from June 2019 to June 2022 were randomly divided into control group(n=34,treated with laparoscopic surgery)and observation group(n=34,treated with laparoscopic surgery plus gonadotropin-releasing hormone analogues),and the therapeutic effects of the two groups were compared.Results The effective rate of the observation group was 97.06%,higher than that of the control group(76.47%),and the difference was statistically significant(P＜0.05).After treatment,the levels of estradiol(E2),follicle-stimulating hormone(FSH),progesterone(P)and luteinuclein(LH)in the observation group were(44.26±11.48)pmol/L,(6.74±1.49)U/L,(4.54±0.93)pmol/L and(7.33±1.91)IU/L,respectively,which were significantly better than those of the control group(67.83±14.57)pmol/L,(18.15±3.72)U/L,(3.26±0.54)pmol/L,(11.15±1.82)IU/L,the difference was statistically significant(P＜0.05).After treatment,the number of sinus follicles in the observation group(8.93±1.24)was higher than that in the control group(7.36±1.18),and the ovarian volume(8.02±1.23)cm3 was lower than that in the control group(9.55±1.37)cm3,the difference was statistically significant(P＜0.05).After 1 year of follow-up,there was no significant difference in menstrual time and menstrual cycle between the two groups(P＞0.05).The pregnancy rate of observation group(67.65%)was significantly higher than that of control group(38.24%),and the recurrence rate(5.88%)was lower than that of control group(17.65%),the difference was statistically significant(P＜0.05).Conclusion In the treatment of chocolate cyst of ovary,laparoscopic surgery combined with gonadotropin-releasing hormone analogues has a remarkable therapeutic effect,which can effectively improve the ovarian function of patients,increase the pregnancy rate and reduce the recurrence rate.
