摘要
目的 探讨利多卡因凝胶+氯己定含漱液+康复新液3 种药物联合治疗口腔溃疡的临床效果.方法 选取2020 年 6月至 2021 年 6 月淄博市妇幼保健院收治的口腔溃疡患者 86 例作为研究对象,按照随机数字表法分为观察组(利多卡因凝胶+氯己定含漱液+康复新液)、对照组(利多卡因凝胶+氯己定含漱液),各 43 例.比较两组患者临床疗效,T细胞淋巴亚群(CD3+、CD4+、CD4+/CD8+),炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)],实验室指标[转化生长因子-β1(TGF-β1)、Smad 同源物 7(Smad7)],口腔菌群(链球菌、韦荣菌、奈瑟氏菌),溃疡愈合时间和疼痛程度[视觉模拟评分法(VAS)评分],统计两组用药期间不良反应发生情况及随访6 个月复发情况.结果 观察组治疗有效率明显高于对照组(P＜0.05).治疗7 d后,两组CD3+、CD4+、CD4+/CD8+、链球菌、韦荣菌水平明显高于治疗前,且观察组明显高于对照组,两组患者IL-6、TNF-α、TGF-β1、Smad7 mRNA水平明显低于治疗前,且观察组明显低于对照组,差异有统计学意义(P＜0.05);治疗前和治疗7 d后两组奈瑟氏菌水平比较差异无统计学意义(P＞0.05);观察组溃疡愈合时间短于对照组,治疗3、7 d后VAS评分低于对照组,差异有统计学意义(P＜0.05);两组均无不良反应发生,且复发率比较差异无统计学意义(P＞0.05).结论 利多卡因凝胶+氯己定含漱液+康复新液治疗口腔溃疡具有良好临床疗效,可有效减轻炎症反应,改善口腔菌群,促进溃疡愈合,降低疼痛程度.
Abstract
Objective To explore the clinical effect of lidocaine gel + chlorhexidine gargarisma + Kangfuxin solution in the treatment of oral ulcer.Methods A total of 86 patients with oral ulcer admitted to Zibo Maternal and Child Health Hospital from June 2020 to June 2021 were selected as the study objects,and were divided into observation group(lidocaine gel + chlorhexidine gargle + Kangfuxin solution)and control group(lidocaine gel + chlorhexidine gargle)according to random number table method,with 43 cases each.The clinical efficacy,T cell lymphoid subsets(CD3+,CD4+,CD4+/CD8+),inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],laboratory indicators[transforming growth factor-β1(TGF-β1),Smad homology 7(Smad7)],Oral microflora(streptococcus,Veillonella,Neisseria),ulcer healing time and pain degree[visual analogue scale(VAS)score]of the two groups were compared,the occurrence of adverse reactions during the treatment period and recurrence at 6 months follow-up of the two groups were analyzed.Results The effective rate of the observation group was significantly higher than that of the control group(P＜0.05).After 7 days of treatment,the levels of CD3+,CD4+,CD4+/CD8+,streptococcus and Veillonia in both groups were significantly higher than before treatment,and the observation group was significantly higher than the control group,the levels of IL-6,TNF-α,TGF-β1 and Smad7 mRNA in both groups were significantly lower than before treatment,and the observation group was significantly lower than the control group,the difference was statistically significant(P＜0.05)).There was no significant difference in Neisseria levels between the two groups before treatment and 7 days after treatment(P＞0.05).The ulcer healing time of the observation group was shorter than that of the control group,and the VAS score after 3 and 7 days of treatment was lower than that of the control group,the difference was statistically significant(P＜0.05).There was no adverse reaction between the two groups,and there was no significant difference in recurrence rate(P＞0.05).Conclusion Lidocaine gel + chlorhexidine gargle + Kangxin liquid has good clinical efficacy in the treatment of oral ulcer,which can effectively reduce inflammation,improve oral flora,promote ulcer healing and reduce pain.
