中医定向透药联合穴位敷贴在重症监护室机械通气胃肠功能障碍患者中的应用价值

摘要
目的 探讨中医定向透药联合穴位敷贴在重症监护室(ICU)机械通气胃肠功能障碍患者中的应用价值.方法 选取2020 年1 月至2022 年12 月东台市中医院ICU收治的机械通气胃肠功能障碍患者82 例作为研究对象,按随机数字表法分为对照组与观察组,各41 例.对照组实施胃肠功能障碍的常规干预,观察组在对照组基础上予以中医定向透药联合穴位敷贴治疗.治疗后评估患者胃肠功能,记录胃肠功能恢复时间并评估患者营养指标(白蛋白及总蛋白)水平.结果 治疗后,观察组急性胃肠损伤(AGI)分级为Ⅰ级患者所占比例高于对照组(P<0.05).观察组肛门排气、肠鸣音恢复以及恢复进食时间短于对照组(P<0.05).治疗后,观察组白蛋白及总蛋白水平高于对照组(P<0.05).结论 ICU 机械通气胃肠功能障碍患者采用中医定向透药、穴位敷贴联合治疗可促进胃肠功能恢复,改善营养状况.
Abstract
Objective To explore the application value of traditional Chinese medicine directional penetration combined with acupoint application in patients with mechanical ventilation gastrointestinal dysfunction in intensive care unit(ICU).Methods A total of 82 patients with mechanical ventilation admitted to ICU of Dongtai Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected as the observation objects,and were divided into control group and observation group according to random number table method,with 41 cases in each group.The control group received routine intervention for gastrointestinal dysfunction,and the observation group was treated with traditional Chinese medicine directional penetration combined with acupoint application on the basis of the control group.After treatment,the gastrointestinal function of patients was assessed,the recovery time of gastrointestinal function was recorded,and the levels of nutritional indicators(albumin and total protein)were assessed.Results After treatment,the proportion of patients with AGI grade Ⅰ in observation group was higher than that in control group(P<0.05).The recovery time of anal exhaust,intestinal ringing and feeding in observation group was shorter than that in control group(P<0.05).After treatment,the albumin and total protein levels in observation group were higher than those in control group(P<0.05).Conclusion The combination treatment of traditional Chinese medicine and acupoint application for gastrointestinal dysfunction in ICU patients with mechanical ventilation can promote the recovery of gastrointestinal function and improve the nutritional status of patients.
