摘要
目的 探讨戈舍瑞林联合桂枝茯苓胶囊在腹腔镜下卵巢巧克力囊肿剥除术后患者中的应用效果.方法 选取2016年 5 月至 2021 年 12 月新泰市中医医院收治的 118 例行腹腔镜手术清除卵巢巧克力囊肿患者作为研究对象,随机分为对照组与观察组,每组59 例.对照组患者术后给予孕三烯酮胶囊,观察组患者术后采用戈舍瑞林联合桂枝茯苓胶囊治疗.比较两组患者临床症状(月经异常、白带异常、小腹坠痛、腰痛)改善时间、临床疗效,治疗前后血清性激素[雌二醇(E2)、黄体生成素(LH)、卵泡刺激素(FSH)]、卵巢储备功能[基础窦卵泡数(AFC)、卵巢体积(OV)、抗苗勒氏管激素(AMH)]、卵巢基质动脉血流动力学参数[搏动指数(PI)、阻力指数(RI)、峰值收缩期速度(PSV)]变化.结果 观察组月经异常、白带异常、腰痛改善时间明显短于对照组(P<0.05),两组小腹坠痛改善时间比较差异无统计学意义(P>0.05);观察组治疗有效率显著高于对照组(P<0.05).治疗后,两组血清 E2 水平及 RI 降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组血清性激素(LH、FSH)水平、卵巢储备功能(AFC、OV、AMH)、卵巢基质动脉血流动力学参数(PI、PSV)升高,且观察组高于对照组,差异有统计学意义(P<0.05).结论 腹腔镜下卵巢巧克力囊肿剥除术后患者采用戈舍瑞林与桂枝茯苓胶囊联合治疗,能够有效改善性激素水平与卵巢储备功能,缓解月经异常等临床症状,促进早日康复.
Abstract
Objective To explore the effect of Goserelin combined with Guizhi poria capsule in the patients with ovarian chocolate cyst after laparoscopy.Methods A total of 118 patients treated in Xintai Hospital of Traditional Chinese Medicine of Tai'an City from May 2016 to December 2021 who underwent laparoscopic surgery to remove chocolate cyst of the ovary were selected as the study objects,and randomly divided into control group and observation group,with 59 cases in each group.The control group was given pregnrienone capsule after operation,and the observation group was given goserelin combined with Guizhi poria capsule after operation.Clinical symptoms improvement time(abnormal menstruation,abnormal leucorrhea,lower abdominal pain,low back pain)were compared between the two groups.Serum sex hormones[estradiol(E2),luteinizing hormone(LH),follicle-stimulating hormone(FSH)],ovarian reserve function[basal sinus follicle number(AFC),ovarian volume(OV),anti-Mullerian hormone(AMH)],dynamic parameters of ovarian stroma arterial blood flow[pulse index(PI),resistance index(RI),peak systolic velocity Degree(PSV)]before treatment and after treatment were compared between the two groups.Results The improvement time of abnormal menstruation,abnormal leucorrhea and low back pain in the observation group was significantly shorter than that in the control group(P<0.05),and there was no significant difference in the improvement time of lower abdominal drop pain between the two groups(P>0.05).The effective rate of treatment in observation group was significantly higher than that in control group(P<0.05).After treatment,serum E2 level and RI in 2 groups were decreased,and observation group was lower than control group,the difference was statistically significant(P<0.05).Serum sex hormone(LH,FSH)levels,ovarian reserve function(AFC,OV,AMH),and ovarian matrix arterial hemodynamics parameters(PI,PSV)were increased in 2 groups,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion The combination therapy of Goserelin and Guizhi Poria capsule can effectively improve the level of sex hormone and ovarian reserve function,relieve the clinical symptoms such as menstrual abnormalities,and promote postoperative rehabilitation in the patients with ovarian chocolate cyst cleared by laparoscopic surgery.
