阿立哌唑联合重复经颅磁刺激治疗Tourette综合征的临床疗效
Clinical Efficacy of Aripiprazole combined with Repeated Transcranial Magnetic Stimulation in the Treatment of Tourette Syndrome

摘要
目的 探讨阿立哌唑联合重复经颅磁刺激(rTMS)治疗Tourette综合征的临床疗效.方法 选取 2022 年 1 月至2023 年 1 月泰安市妇幼保健院收治的Tourette综合征患儿 120 例作为研究对象,按照随机数字表法分为对照组和观察组,每组60 例.对照组予以阿立哌唑,观察组采用阿立哌唑联合rTMS治疗.比较两组临床疗效,治疗前后症状积分、血清神经递质水平及不良反应发生率.结果 观察组治疗有效率为91.67%,高于对照组的76.67%(P＜0.05).治疗后,观察组耶鲁综合抽动严重程度量表(YGTSS)中抽动类型、抽动频率、抽动强度、复杂程度、干扰程度评分及总分均低于对照组(P＜0.05).治疗后,观察组血清神经递质 5-羟色胺(5-HT)、多巴胺(DA)、去甲肾上腺素(NE)水平低于对照组,谷氨酸(Glu)水平高于对照组,差异有统计学意义(P＜0.05).观察组不良反应发生率为 11.67%,对照组为 10.00%,差异无统计学意义(P＞0.05).结论 阿立哌唑联合rTMS治疗Tourette综合征可提高临床疗效,有效改善患儿临床症状,调节血清神经递质水平,且安全性较高.
Abstract
Objective To explore the clinical effect of aripiprazole combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of Tourette syndrome.Methods A total of 120 children with Tourette syndrome admitted to Tai'an Maternal and Child Health Hospital from January 2022 to January 2023 were selected as the study objects,and were divided into control group and observation group according to random number table method,with 60 cases in each group.Control group was treated with aripiprazole,observation group was treated with aripiprazole combined with rTMS.The clinical efficacy,symptom score,serum neurotransmitter level and incidence of adverse reactions were compared between the two groups.Results The effective rate of observation group was 91.67%,which was higher than that of control group 76.67%(P＜0.05).After treatment,the scores of twitch type,twitch frequency,twitch intensity,complexity,disturbance degree and total score of YGTSS in observation group were lower than those in control group(P＜0.05).After treatment,the levels of serum neurotransmitters 5-hydroxytryptamine(5-HT),dopamine(DA)and norepinephrine(NE)in observation group were lower than those in control group,and glutamate(Glu)was higher than those in control group,with statistical significance(P＜0.05).The incidence of adverse reactions was 11.67%in the observation group and 10.00%in the control group,with no statistical significance(P＞0.05).Conclusion Aripiprazole combined with rTMS in the treatment of Tourette syndrome can improve clinical efficacy,effectively improve clinical symptoms,regulate serum neurotransmitter levels,and have high safety.
