麦角新碱联合卡前列素氨丁三醇用于预防二次剖宫产产后出血的价值

摘要
目的 探讨麦角新碱联合卡前列素氨丁三醇用于预防二次剖宫产产后出血的效果.方法 选取2022 年4 月至2023年 4 月南通大学附属泰州妇产医院收治的 70 例二次剖宫产产妇作为研究对象,按照随机数字表法分为对照组和观察组,各35例.对照组予以卡前列素氨丁三醇联合缩宫素,观察组采用麦角新碱联合卡前列素氨丁三醇,比较两组预防产后出血的效果.结果 观察组手术时间短于对照组,术中出血量、术后2 h出血量、术后24 h出血量少于对照组,差异有统计学意义(P＜0.05);观察组治疗有效率高于对照组,差异有统计学意义(P＜0.05);治疗后,观察组输血量少于对照组,止血所需时间、住院时间短于对照组,差异有统计学意义(P＜0.05);治疗后,观察组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)短于对照组,D-二聚体(D-D)低于对照组,差异有统计学意义(P＜0.05);治疗后,观察组舒张压、收缩压、心率低于对照组(P＜0.05).结论 麦角新碱联合卡前列素氨丁三醇用于预防二次剖宫产产后出血效果更显著,不仅可控制术中以及术后出血量,还可缩短住院时间,同时可将血压以及心率控制在合理范围内,改善凝血功能,降低产后出血发生率.
Abstract
Objective To explore the effect of ergotrine combined with carprost trobutrin in preventing hemorrhage after second caesarean section.Methods Seventy women with second cesarean section admitted to Taizhou Obstetrics and Gynecology Hospital Affiliated to Nantong University from April 2022 to April 2023 were selected as the research objects,and were divided into control group and observation group according to random number table method,with 35 cases in each group.The control group was given carprost tramine combined with oxytocin,and the observation group was given ergotrine combined with carprost tramine.The prevention effect of postpartum hemorrhage between the two groups was compared.Results The operation time of the observation group was shorter than that of the control group,and the amount of blood loss during operation,2 h after operation,and 24 h after operation was less than that of the control group,the difference was statistically significant(P＜0.05).The effective rate of treatment in the observation group was higher than that in the control group,the difference was statistically significant(P＜0.05).After treatment,the blood transfusion volume in the observation group was less than that in the control group,and the time required for hemostasis and hospital stay were shorter than that in the control group,with statistical significance(P＜0.05).After treatment,the prothrombin time(PT)and activated partial thrombin time(APTT)in observation group were shorter than those in control group,and D-dimer(D-D)was lower than that in control group,the difference was statistically significant(P＜0.05).After treatment,the diastolic blood pressure,systolic blood pressure and heart rate in the observation group were lower than those in the control group(P＜0.05).Conclusion The effect of ergotrine combined with carprost tramine on the prevention of postpartum hemorrhage after secondary cesarean section is more significant,which can not only control the amount of intraoperative and postoperative blood loss,but also shorten the length of hospital stay,control blood pressure and heart rate within a reasonable range,improve coagulation function and reduce the incidence of postpartum hemorrhage.
