麦角新碱联合卡贝缩宫素对高龄剖宫产产后出血产妇应激指标及宫底高度的影响
Effects of Ergostine combined with Carbetin on Stress Indexes and Fundus Height in Elderly Women with Postpartum Hemorrhage after Cesarean Section

摘要
目的 探讨麦角新碱联合卡贝缩宫素对高龄剖宫产产后出血产妇应激指标及宫底高度的影响.方法 选取2021 年3 月至2023 年2 月泰安市妇幼保健院收治的108 例高龄剖宫产产后出血产妇作为研究对象,随机分为对照组与观察组,各54 例.对照组给予卡贝缩宫素,观察组采用卡贝缩宫素+麦角新碱治疗,比较两组的治疗效果.结果 观察组治疗有效率高于对照组,差异有统计学意义(P＜0.05);治疗后,观察组纤维蛋白原(FIB)水平较高,凝血酶原时间(PT)、部分凝血活酶时间(APTT)水平较短,差异有统计学意义(P＜0.05);用药 24 h后,观察组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、脂质过氧化物(LPO)水平较低,谷胱甘肽过氧化物酶(GSH-Px)水平较高,差异有统计学意义(P＜0.05);用药2 h后,观察组收缩压、舒张压、宫底高度、心率低于对照组,差异有统计学意义(P＜0.05).结论 麦角新碱联合卡贝缩宫素用于高龄剖宫产产后出血产妇效果理想,能快速降低应激指标、宫底高度,控制心率与血压.
Abstract
Objective To explore the effects of ergocline combined with carbetocin on stress index and fundus height of elderly women with hemorrhage after cesarean section.Methods A total of 108 elderly women with postpartum hemorrhage after cesarean section treated in Tai'an Maternal and Child Health Hospital from March 2021 to February 2023 were selected as the study objects,and randomly divided into control group and observation group,with 54 cases in each group.The control group was given carbetocin,the observation group was treated with carbetocin + ergotrine,and the therapeutic effect of the two groups was compared.Results The effective rate of treatment in the observation group was higher than that in the control group,the difference was statistically significant(P＜0.05).After treatment,the fibrinogen(FIB)level in observation group was higher,and the prothrombin time(PT)and partial thrombin time(APTT)were shorter,the difference was statistically significant(P＜0.05).After 24 h of treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and lipid peroxidase(LPO)in observation group were higher than those of the low level of tathione peroxidase(GSH-Px),the difference was statistically significant(P＜0.05).After 2 h of treatment,systolic blood pressure,diastolic blood pressure,fundal height and heart rate in observation group were lower than those in control group,and the differences were statistically significant(P＜0.05).Conclusion Ergotrine combined with carbetocin can be used to reduce stress index,fundus height and control heart rate and blood pressure in elderly women with hemorrhage after cesarean section.
