银杏酮酯分散片联合硫酸氢氯吡格雷片治疗缺血性脑卒中的成本-效果

摘要
目的 评估银杏酮酯分散片联合硫酸氢氯吡格雷片相比于硫酸氢氯吡格雷片治疗缺血性脑卒中的成本-效果,为银杏酮酯分散片联合硫酸氢氯吡格雷片的临床用药提供决策证据.方法 从医疗卫生体系角度出发,采用Microsoft Excel 2019软件构建决策树模型,模拟银杏酮酯分散片联合硫酸氢氯吡格雷片相比于硫酸氢氯吡格雷片治疗缺血性脑卒中的总成本和临床产出,并进行增量分析和单因素敏感性分析.其中成本数据来源于相关价格网站,疗效数据来源于文献.结果 基础分析结果显示,银杏酮酯分散片联合硫酸氢氯吡格雷片相比于硫酸氢氯吡格雷片成本低(1 656 元vs.2 795 元),效果好(再次卒中发生率低,2.22%vs.10.00%),为绝对优势方案.单因素敏感性分析显示,对结果影响最大的前3 位参数分别是:硫酸氢氯吡格雷片疗效、银杏酮酯分散片价格和银杏酮酯分散片联合硫酸氢氯吡格雷片疗效.结论 银杏酮酯分散片联合硫酸氢氯吡格雷片相比于硫酸氢氯吡格雷片治疗缺血性脑卒中成本低,效果好,为绝对优势方案.
Abstract
Objective To evaluate the cost-effectiveness of Ginkgo Biloba Terpene Lactones dispersive tablets combined with Clopidogrel Bisulfate tablets compared with Clopidogrel Bisulfate tablets in the treatment of ischemic stroke,and to provide decision-making evidence for the clinical use of Ginkgo Biloba Terpene Lactones dispersive tablets combined with Clopidogrel Bisulfate tablets.Methods From the perspective of healthcare system,Microsoft Excel 2019 software was used to build a decision tree model to simulate the total cost and clinical output of Ginkgo Biloba Terpene Lactones Dispersive tablets combined with Clopidogrel Bisulfate tablets compared with Clopidogrel Bisulfate tablets in the treatment of ischemic stroke,and incremental analysis and single factor sensitivity analysis were conducted.The cost data came from the relevant price website,and the efficacy data came from the literature.Results The basic analysis results show that the combination of Ginkgo Biloba Terpene Lactones Dispersible tablets with Clopidogrel Bisulfate tablets is less costly(CNY 1,56 vs.CNY 2 795)and more effective(lower recurrent stroke incidence,2.22%vs.10.00%),making it an absolutely superior option.Univariate sensitivity analysis indicates that the three most influential parameters on the results are the efficacy of Clopidogrel Bisulfate tablets,the price of Ginkgo Biloba Terpene Lactones Dispersible tablets,and the combined efficacy of Ginkgo Biloba Terpene Lactones Dispersible tablets with Clopidogrel Bisulfate tablets.Conclusion Compared with Clopidogrel Bisulfate tablets,Ginkgo Biloba Terpene Lactones dispersive tablets combined with Clopidogrel Bisulfate tablets have lower cost and better effect in the treatment of ischemic stroke,representing an absolutely superior option that is worth promoting.
