依那西普联合倍他米松关节腔注射治疗强直性脊柱炎的效果

摘要
目的 探讨强直性脊柱炎采用依那西普联合倍他米松关节腔注射方式治疗的临床疗效.方法 选取2020 年1 月至2023 年 12 月于苏州大学第一附属人民医院就诊的 100 例强直性脊柱炎患者为研究对象,采用随机数字表法分为对照组与观察组,各50 例.对照组予以关节腔内注射倍他米松治疗,观察组采用关节腔注射依那西普联合倍他米松治疗,比较两组的治疗效果.结果 治疗后观察组腰椎骨密度、骨钙素、骨形态发生蛋白-2 高于对照组,骨碱性磷酸酶低于对照组,差异有统计学意义(P＜0.05);治疗后观察组疲乏、晨僵,中轴、外周关节疼痛及肌腱端痛评分高于对照组(P＜0.05);治疗后观察组疼痛程度、关节肿胀程度评分及压痛分级低于对照组(P＜0.05);观察组治疗有效率高于对照组(P＜0.05).结论 对强直性脊柱炎患者实施依那西普联合倍他米松关节腔注射治疗,可有效改善骨代谢、腰椎骨密度及躯体功能,减轻临床症状,效果显著.
Abstract
Objective To explore the clinical efficacy of joint cavity injection of etanercept combined with betamethasone in the treatment of ankylosing spondylitis.Methods A total of 100 patients with ankylosing spondylitis treated in the First Affiliated People's Hospital of Soochow University from January 2020 to December 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with 50 cases in each group.The control group was treated with intraarticular injection of betamethasone,and the observation group was treated with intraarticular injection of etanercept combined with betamethasone.The therapeutic effect of the two groups was compared.Results After treatment,lumbar bone density,osteocalcin and bone morphogenetic egg-2 in observation group were higher than those in control group,and bone alkaline phosphatase was lower than that in control group,the difference was statistically significant(P＜0.05).After treatment,the scores of fatigue,morning stiffness,axial and peripheral joint pain and tendon end pain in observation group were higher than those in control group(P＜0.05).After treatment,the pain degree,joint swelling degree and tenderness grade of the observation group were lower than those of the control group(P＜0.05).The effective rate of the observation group was higher than that of the control group(P＜0.05).Conclusion The joint injection of etanercept and betamethasone in patients with ankylosing spondylitis can effectively improve bone metabolism,lumbar bone density and physical function,and alleviate clinical symptoms.
