基于升阳举陷理论运用补中益气汤临证医案举隅

摘要
补中益气汤出自《脾胃论》,为李东垣所创的代表方剂之一,具有补中益气、升阳举陷功效,临床上常用于治疗脾胃气虚、清阳不升、气虚下陷、气虚发热诸证.本文通过文献研究与临床实践相结合总结出根据升阳举陷理论运用补中益气汤的经验,并对典型医案进行探析,如水肿、月经不调、糖尿病受外伤后伤口不愈等辨证属于脾胃气虚、清阳不升之证,皆用补中益气汤加减治疗,体现了辨证论治,异病同治的中医诊疗思路.
Abstract
Buzhong Yiqi Decoction,derived from Spleen and Stomach Theory,is one of the representative prescriptions created by Li Dongyuan.It has the effect of tonifying the middle and supplementing qi,raising and sinking the yang.It is commonly used clinically to treat spleen and stomach qi deficiency,failing to raise the qing yang,sinking the qi deficiency,and fever due to qi deficiency.Through the combination of literature research and clinical practice,this paper summarizes the experience of using Buzhong Yiqi Decoction according to the theory of rising yang and lifting depression,and analyzes typical medical cases,such as edema,irregular menstruation,diabetes wounds that do not heal after trauma,which belong to the syndrome of spleen stomach qi deficiency and rising yang,and all are treated with Buzhong Yiqi Decoction,which reflects the TCM diagnosis and treatment idea of treating different diseases according to syndrome differentiation.
