三草尿毒灵治疗脾肾气虚型慢性肾脏病的临床疗效

摘要
目的 探讨三草尿毒灵治疗脾肾气虚型慢性肾脏病(CKD)的临床疗效.方法 选取 2022 年 6 月至 2023 年 2 月于山东省慢性病医院(山东省康复中心)中医科就诊的100 例脾肾气虚型CKD患者为研究对象,随机分为观察组与对照组,各 50 例,观察组 4 例和对照组 4 例在治疗期间失访且未复诊.观察组使用基础治疗+三草尿毒灵中药煎剂,对照组使用基础治疗+多糖铁复合物胶囊,比较两组的治疗效果.结果 治疗后,两组尿素氮(BUN)、铁调素(Hepcidin)降低,估算肾小球滤过率(eGFR)、血红蛋白(Hb)、血清铁(SI)升高,且观察组改善效果优于对照组,差异有统计学意义(P＜0.05);观察组治疗肾性贫血疗效优于对照组(P＜0.05);观察组中医证候疗效优于对照组(P＜0.05);两组治疗前后血清白蛋白(ALB)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、γ-谷氨酰转移酶(γ-GGT)比较差异无统计学意义(P＞0.05),均在正常范围内;观察组和对照组均没有出现任何不良反应.结论 三草尿毒灵可有效提高脾肾气虚型CKD患者的GFR、Hb、SI水平,降低血清Hepcidin水平,改善中医证候,且安全性较高.
Abstract
Objective To explore the clinical effect of Sancaouridine on chronic kidney disease(CKD)with spleen-kidney qi deficiency.Methods A total of 100 patients with spleen-kidney qi deficiency CKD who were treated in the Department of Traditional Chinese Medicine of Shandong Chronic Disease Hospital(Shandong Rehabilitation Center)from June 2022 to February 2023 were selected as the study objects,and were randomly divided into observation group and control group,with 50 cases in each group,4 cases in the observation group and 4 cases in the control group.Observation group was treated with basic treatment+Sancaouridine decoction,control group was treated with basic treatment+polysaccharide iron complex capsule,and the therapeutic effect of the two groups was compared.Results After treatment,urea nitrogen(BUN)and iron regulation hormone(Hepcidin)were decreased,estimated glomerular filtration rate(eGFR),hemoglobin(Hb)and serum iron(SI)were increased,and the improvement effect of observation group was better than that of control group,the difference was statistically significant(P＜0.05).The treatment effect of the observation group was better than that of the control group(P＜0.05).The curative effect of TCM syndrome in observation group was better than that in control group(P＜0.05).There was no significant difference in serum albumin(ALB),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and gamma-glutamyltransferase(gamma-GGT)between the two groups before and after treatment(P＞0.05),and all were within the normal range.Neither the observation group nor the control group had any adverse reactions.Conclusion Sancauridulin can effectively improve the levels of GFR,Hb and SI,reduce the level of serum Hepcidin and improve the TCM syndromes in patients with spleen-kidney qi deficiency type CKD,with high safety.
