祛湿止痒方治疗湿热浸淫型急性湿疹的临床疗效

摘要
目的 探讨祛湿止痒方治疗湿热浸淫型急性湿疹的临床疗效及对患者炎症因子和免疫功能的影响.方法 选取2021 年1 月至2022 年12 月于大连市中医医院皮肤科就诊的156 例湿热浸淫型急性湿疹患者作为研究对象,随机分为观察组与对照组,各78 例.对照组予以常规西医治疗,观察组在对照组基础上口服祛湿止痒方,比较两组的治疗效果.结果 治疗后,两组瘙痒评分、湿疹面积和严重程度指数(EASI)评分和皮肤病生活质量指数(DLQI)评分降低,且观察组低于对照组,差异有统计学意义(P＜0.05);观察组治疗有效率高于对照组(P＜0.05);治疗后两组肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)、免疫球蛋白E(IgE)和嗜酸性粒细胞水平降低,且观察组低于对照组,差异有统计学意义(P＜0.05);两组患者治疗期间均无不良反应发生;随访 6 个月,观察组复发率低于对照组(P＜0.05).结论 祛湿止痒方治疗湿热浸淫型急性湿疹临床疗效显著,可有效抑制炎症反应,改善免疫功能,具有较低的复发率和较高的安全性.
Abstract
Objective To explore the clinical efficacy of Qushi Zhiitching formula in treating acute eczema of damp-heat immersion and its influence on inflammatory factors and immune function.Methods A total of 156 patients with damp-heat immersion acute eczema who were treated in the dermatology department of Dalian Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were selected as the study objects and randomly divided into observation group and control group,with 78 cases in each group.The control group was given conventional Western medicine treatment,and the observation group was given oral Qushi Zhiprong prescription based on the control group.The therapeutic effect of the two groups was compared.Results After treatment,itch score,eczema area and severity index(EASI)score and skin disease Quality of Life index(DLQI)score of two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P＜0.05).The effective rate of the observation group was higher than that of the control group(P＜0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-4(IL-4),immunoglobulin E(IgE)and eosinophils in 2 groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P＜0.05).No adverse reactions occurred in both groups during treatment.After 6 months of follow-up,the recurrence rate of observation group was lower than control group(P＜0.05).Conclusion Qushi Zhiitching formula has significant clinical efficacy in the treatment of acute eczema caused by dampness and heat.It can effectively inhibit inflammatory response,improve immune function,and has a low recurrence rate and high safety.
