电话访谈法在门诊筛查苯二氮类药物依赖的效果分析

摘要
目的 本研究创新性地将电话访谈法与结构式访谈相结合,在保障一对一有效访谈的基础上降低时间等相关成本,并探讨基于结构式访谈纲要的电话访谈法在门诊筛查苯二氮类药物(benzodiazepines,BZDs)依赖的效果.方法 在门诊处方BZDs的人群中应用平均日最大量(average daily maximum dose,ADMD)筛查出可疑药物依赖人群,在该人群中利用随机数字表法抽取40例,进行基于结构式访谈纲要的电话访谈,并预约现场评估及诊断.对其应用苯二氮类药物依赖自评量表(Benzodiazepine Dependence Questionnaire,BDEPQ)进行评估,并用国际疾病分类第 10 版(International Classification of Diseases 10th Revision,ICD-10)进行现场诊断.运用 Kappa 检验比较基于结构式访谈纲要的电话访谈法与医生诊断、量表评估之间的一致性并进行分析,用Fisher确切概率法评估其差异性.结果 基于结构式访谈纲要的电话访谈法与BDEPQ量表评估的一致性较好(Kappa值=0.773),阳性符合率为96.55％,阴性符合率为77.78％,总符合率为92.11％,二者的阳性诊出率经Fisher确切概率法比较差异无统计学意义(P＞0.05);与ICD-10的现场诊断一致性较好(Kappa值=0.751),阳性符合率为93.55％,阴性符合率为85.71％,总符合率为92.11％,阳性诊出率经Fisher确切概率法比较无显著差异(P＞0.05).结论 基于结构式访谈纲要的电话访谈法能有效筛查出BZDs依赖患者,且方法简便,值得推广.
Abstract
Objective This study innovatively combined telephone interview using structured interview to reduce time and other related costs on the basis of ensuring effective one-on-one interview,and investigate the effect of telephone interview method based on structured interview outline in benzodiazepines dependence screening in the outpatient department.Methods In this study,average daily maximum dose(ADMD)was used to screen out the suspected drug dependence population in the people who prescribed the benzodiazepines tablets in outpatient clinic,and 40 patients were selected from this population by random number table method for telephone interview based on structured interview outline,and on-site assessment and diagnosis.The patients were evaluated by Benzodiazepines Dependence Questionnaire(BDEPQ)and diagnosed by International Classification of Diseases 10th Revision(ICD-10).Kappa test was used to compare and analyze the consistency among standardized interview outline and doctor's diagnosis and scale evaluation,and Fisher's exact probability method was used to evaluate the differences.Results The telephone interview method based on the structured interview outline had a good consistency with BDEPQ scale evaluation(Kappa=0.773),with a positive coincidence rate of 96.55％,a negative coincidence rate of 77.78％,and a total coincidence rate of 92.11％.There was no significant difference in the positive detection rate between the two groups by Fisher's exact probability method(P＞0.05).It is also in good agreement with ICD-10(Kappa=0.751).The positive coincidence rate was 93.55％,the negative coincidence rate was 85.71％,and the total coincidence rate was 92.11％.There was no significant difference in the positive detection rate between them by Fisher's exact probability method(P＞0.05).Conclusion The telephone interview method based on structured interview outline can effectively screen out the BZDs-dependent patients,which is a simple method and is worth popularizing.
