摘要

目的 采用磁共振波谱(Magnetic Resonance Spectrum,MRS)技术研究酒精依赖患者代谢物变化特点,并探讨其与认知损害之间的关系,以期发现酒精依赖患者认知损害的神经生物学机制.方法 使用重复性成套神经心理状态测验(Repeatable Battery for the Assessment of Neuropsychological Status,RBANS)对 60 例住院酒精依赖患者和40例健康志愿者的认知功能进行评估,采用MRS观测酒精依赖者额叶、杏仁核关键脑区代谢物变化,并分析MRS与RBANS之间的相关性.结果 与正常对照组比较,酒精依赖组在左侧额叶、右侧额叶和左侧杏仁核NAA/Cr值均显著降低(P<0.05),左侧额叶、右侧额叶、右侧杏仁核脑区Cho/Cr值显著降低(P<0.05),右侧额叶脑区Glu/Cr值显著降低,而左侧杏仁核区Glu/Cr显著增高(P<0.05).酒精依赖患者即刻记忆及注意功能分值与左侧额叶NAA/Cr值及右侧杏仁核Cho/Cr值呈正相关(P<0.05);言语功能分值与左侧额叶Cho/Cr值呈正相关(P<0.05);而左侧杏仁核Glu/Cr与即刻记忆分值呈负相关(P<0.05).结论 酒精依赖者双侧额叶及杏仁核代谢产物异常,可能是酒精依赖者认知功能损害的神经生物学基础.

Abstract

Objective Using magnetic resonance spectroscopy(MRS)to study the characteristics of metabolites in alcohol-dependent patients,explore the relationship between metabolites and cognitive impairment and to discover the neurobiological mechanism of cognitive impairment in alcohol-dependent patients.Methods Using repeatable battery for the assessment of neuropsychological status(RBANS)to assess cognitive function of 60 hospitalized alcohol-dependent patients and 40 healthy volunteers,and MRS was used to observe metabolite changes in the frontal lobes and amygdala of alcohol-dependent patients,then the correlation between MRS and RBANS was analyzed.Results Compared with control group,the NAA/Cr values of the left frontal lobe,right frontal lobe and left amygdala in alcohol-dependence group decreased significantly(P<0.05),the Cho/Cr values of left frontal lobe,right frontal lobe and right amygdala decreased significantly(P<0.05),and the Glu/Cr value of right frontal lobe brain region decreased significantly,but that of left amygdala region increased significantly,(P<0.05).Correlation analysis between RBANS and MRS in alcohol-dependence group showed that the scores of immediate memory and attention function were positively correlated with the NAA/Cr value of left frontal lobe and the Cho/Cr value of right amygdala(P<0.05).The score of speech function were positively correlated with the Cho/Cr value of left frontal lobe(P<0.05).While the Glu/Cr value of left amygdala was negatively correlated with immediate attention scores(P<0.05).Conclusion Abnormal metabolites of bilateral frontal lobes and amygdala in alcohol-dependent patients may be the neurobiological basis of cognitive impairment in alcohol-dependent patients.
