基于门急诊患者美敏伪麻口服溶液和复方磷酸可待因口服溶液的药物利用评价

摘要
目的 了解门急诊患者美敏伪麻口服溶液和复方磷酸可待因口服溶液的使用现状,为含麻精药品复方制剂的滥用监测提供依据.方法 基于南京医科大学大数据平台检索处方两种药品的患者信息,采用用药频度(defined daily dose sum,DDDs)和药物利用指数(drug utilization index,DUI)描述,运用 SPSS 26.0 对不同组间构成比进行x2检验.结果 获得处方美敏伪麻口服溶液的患者7964例(男女比为1∶1.29),诊断主要为呼吸道感染;处方复方磷酸可待因口服溶液的患者410例(男女比为1∶1.07),诊断主要为肺癌.美敏伪麻口服溶液2019年DDDs最高,2017-2022年DUI值均低于1,用药基本合理;复方磷酸可待因口服溶液2018年DDDs最高,且2019年、2020年DUI值分别为1.82、1.11,表明存在滥用可能.结论 医疗机构应该重视复方磷酸可待因口服溶液的合理使用和潜在滥用问题,加强这类药品的监管.
Abstract
Objective To understand the drug utilization status of compound pseudoephedrine hydrochloride and codeine phosphate oral solution in outpatients and emergency patients,and provide evidence for the abuse monitoring.Methods Based on the big-data platform of Nanjing Medical University,the information of patients prescribed these two drugs was retrieved,and described by defined daily dose sum(DDDs)and drug utilization index(DUI).Differences of categorical variables between groups were analyzed using x2 test between with SPSS 26.0.Results 7964 patients(male to female ratio:1∶1.29)were prescribed compound pseudoephedrine hydrochloride oral solution,and the corresponding diagnosis was mainly respiratory tract infection;410 patients were prescribed compound codeine phosphate oral solution(male to female ratio:1∶1.07),and the corresponding diagnosis was mainly lung cancer.The DDDs of compound pseudoephedrine hydrochloride oral solution were the highest in 2019,the DUI in 2017-2022 were all lower than 1,and the medication was basically reasonable.The DDDs of compound codeine phosphate oral solution were the highest in 2018,and the DUI in 2019 and 2020 were 1.82 and 1.11,respectively,which suggested possible abuse.Conclusion Medical institutions should pay attention to the rational use and potential abuse of compound codeine phosphate oral solution,and strengthen the supervision of such drugs.
