某院2020年至2023年住院患者麻醉药品的使用情况及合理性分析

摘要
目的 分析某院住院患者麻醉药品的使用情况并评价其合理性,为麻醉药品的科学管理和临床合理应用提供参考和依据.方法 收集2020年10月—2023年10月某院住院患者麻醉药品的使用数据,对其使用量、销售金额、用药频度(DDDs)及其排序、限定日费用(DDC)及其排序、排序比(B/A)进行统计处理和分析.结果 4年来,该院共使用的麻醉药品有9个品种,15个品规,麻醉药品的年销售总金额稳步增长.盐酸羟考酮缓释片(40 mg)和芬太尼透皮贴剂(8.4 mg)的总体用药量较多,盐酸羟考酮缓释片(40 mg)的DDDs排序稳居前2名,B/A基本均是1.0,其同步性较好.注射用盐酸瑞芬太尼的DDDs逐年上升,所有麻醉药品的B/A范围在0.27～3.67之间.结论 该院2020至2023年麻醉药品的临床使用基本合理,但仍存在不足,需进一步加强临床规范化用药.
Abstract
Objective To analyze the current use situation and evaluate the rationality of outpatient narcotic drug application in a hospital,providing a reference for promoting rational drug use and standardized management.Methods Data on the use of narcotic drugs among inpatients at a certain hospital from October 2020 to October 2023 were collected.The quantity used,sales amount,defined daily doses(DDDs)and their ranking,defined daily costs(DDCs)and their ranking,and B/A ratio(ratio of DDD ranking to DDC ranking)were statistical processed and analyzed.Results Over the four-year period,inpatients in the hospital used fifteen drug specifications and nine dosage forms of narcotic drugs.The total consumption of narcotic drugs increased steadily year by year.Oxycodone hydrochloride sustained-release tablets(40 mg)and Fentanyl transdermal patches(8.4 mg)had larger overall dosages,and the DDDs of oxycodone hydrochloride sustained-release tablets(40 mg)ranked in the top 2,with B/A values of 1.0,indicating good synchronization.The DDDs of remifentanil hydrochloride for injection increased annually,and the B/A values of all narcotic drugs ranged from 0.27 to 3.67.Conclusion The clinical use of narcotic drugs in our hospital from 2020 to 2023 is generally reasonable,but there are still shortcomings,necessitating further reinforcement of clinical standardized drug use.
