右美沙芬依赖与双相情感障碍共病一例报告
A case report on comorbidity of dextromethorphan dependence with bipolar disorder

摘要
通过对一例诊断为右美沙芬依赖与双相情感障碍的20次住院患者的分析,建议对右美沙芬滥用和依赖患者要注意他们是否合并双相情感障碍.结合以前报导的"超量右美沙芬致持续严重精神障碍"的病例,推断右美沙芬可快速改善抑郁情绪,但过量会引起躁狂症状.因此,慎重建议采用右美沙芬和情绪稳定剂(如碳酸锂或丙戊酸镁)合用来治疗双相情感障碍的抑郁症状,以达到既可治疗抑郁,又可防止抗抑郁药引起的躁狂转相的作用.
Abstract
Through the analysis of a case involving a 20-time inpatient diagnosed with dextromethorphan dependence and bipolar disorder,it is suggested that attention should be paid to the possibility of comorbid bipolar disorder in patients with dextromethorphan abuse and dependence.Combining previous reports of cases where"excessive dextromethorphan caused persistent severe mental disorders,"it is inferred that dextromethorphan can rapidly improve depressive symptoms but may induce manic symptoms when overused.Therefore,it is cautiously recommended to use dextromethorphan in combination with mood stabilizers(such as lithium carbonate or magnesium valproate)for the treatment of depressive symptoms in bipolar disorder,in order to achieve both therapeutic effects on depression and prevention of antidepressant-induced manic switching.
