Construction and efficacy evaluation of drug treatment management model for hypertensive patients in medical consortia
Objective To establish a consulting platform for drug treatment management(MTM)based on the medical alliance system and investigate the efficacy of this platform management for hypertensive patients with poor medication compliance.Methods A pharmaceutical consultation platform containing elements of drug treatment management was set up in the medical alliance system.A total of 80 discharged hypertensive patients with poor compliance who had adjusted the hypertensive drug treatment plan during hospitalization were selected and randomly divided into the observation group and the control group,wtih 40 in each group.The patients in the control group received traditional medical services while those in the observation group received additional online drug treatment management consulting service.After 3 and 6 months,the percentage of patients with blood pressure reaching the standard was compared between the two groups.Meanwhile,the patient's medication compliance,knowledge of hypertension prevention and treatment and awareness of safe medication were also compared after 6 months.Results At the end of the 3rd and 6th month,the compliance rate of blood pressure in the observation group was remarkably higher than that in the control group(χ2=8.531,4.344,both P<0.05).Six months later,the observation group showed better medication compliance,knowledge of hypertension prevention and treatment and awareness of safe medication than the control(χ2=32.903,9.319,9.319,all P<0.05).The BMI((22.72±3.10)kg∙m-2 vs(23.99±2.17)kg∙m-2,t=12.093,P=0.028),high sodium intake ratio(22.50%(9/40)vs 50.00%(20/20),χ2=20.163,P=0.017),inappropriate alcohol consumption rate(5%(5/40)vs 40%(16/40),χ2=39.904,P<0.001),anxiety score((2.43±1.00)points vs(3.71±1.03)points,t=4.527,P=0.037)and depression score((2.14±0.36)points vs(3.89±1.20)points,t=17.793,P<0.001)of the observation group were all lower than those of the control.However,the moderate physical activity rate of the observation group was higher than that of the control(77.50%(31/40)vs 57.50%(23/40),χ2=21.472,P=0.016).The total score of quality of life was significantly higher in the observation group than in the control((72.63±10.23)points vs(65.28±9.89)points,t=29.198,P<0.001).Conclusion The drug therapy management(MTM)consulting service provided by the platform of medical alliance system can improve the drug compliance and awareness of disease and safe medication to elevate the rate of normal blood pressure in those hypertensive patients with poor compliance.
