With the aggravation of the aging of our society,the care of elderly patients with chronic diseases has become the focus of attention.This paper summarizes the meaning of four-element linkage integrated care for chronic diseases and elderly patients with chronic diseases and its research development process and current situation.In the process of quaternary linkage integrated care for elderly patients with chronic diseases,the hospital can provide professional technical support for integrated care,the community is the core of integrated care,the family provides daily care places for integrated care,and the development of medical and nursing institutions complements the medical care services for elderly patients with chronic diseases.In addition,the application of the four-element linkage integrated care model to the care of elderly patients with chronic diseases was further discussed,which can provide reference for the study of the care of elderly patients with chronic diseases in China.
