心血管疾病患者的日常体力活动时间与最大脂肪氧化的关系
The association between daily physical activity time and maximal fat oxidation in patients with cardiovascular disease

摘要
目的 探讨心血管疾病患者日常体力活动时间和最大脂肪氧化速率的关系.方法 此研究为横断面研究.选取2021年4月至2022年12月北京医院心血管内科进行运动心肺测试的心血管疾病患者,收集临床基本资料和运动心肺数据.测试前使用国际体力活动问卷进行评估.运动心肺测试选择最大脂肪氧化速率时、无氧阈时和峰值3个时段的摄氧量(VO2)、摄氧量占预计值百分比、氧脉搏和通气量/二氧化碳排出量(VE/VCO2)数据.按日常体力活动时间的四分区(P0~P25,>P25~P50,>P50~P75,>P75~P100)进行四分组.比较不同体力活动时间分组间上述指标的差异.根据运动心肺测试的结果,按照Weber心功能分为A、B、C、D四组.比较四组间日常体力活动时间、最大脂肪氧化速率和FAT-max(最大脂肪氧化速率时的VO2占预计最大VO2百分比值)的差别.Pearson简单相关分析日常体力活动时间和最大脂肪氧化速率的相关性.结果 最终共有180例患者纳入此研究,男性100例,女性80例,平均(62.76±10.98)岁.不同日常体力活动时间分组间,最大脂肪氧化速率和FATmax未见差异.最大脂肪氧化速率时的VE/VCO2(P=0.021),无氧阈时的VE/VCO2(P=0.006),峰值VE/VCO2(P<0.001)存在组间差异.不同Weber心功能分组下,最大脂肪氧化速率(P<0.001)、FATmax(P<0.001)、氧脉搏(P<0.001)、最大氧化速率时的VE/VCO2(P=0.002)在各组间存在差异.Pearson相关分析提示,日常体力活动时间与最大脂肪氧化速率(P=0.658)、FATmax(P=0.754)和最大脂肪氧化速率时VE/VCO2(P=0.054)不相关,与无氧阈时VE/VCO2(P=0.033),峰值VE/VCO2(P=0.010)相关.结论 在心血管疾病患者中,日常体力活动时间可能与最大脂肪氧化速率和FATmax不相关,与VE/VCO2有关.不同心功能分级间,最大脂肪氧化速率存在差异.心功能越差,最大脂肪氧化速率越低,出现时间越早.
Abstract
Objective To explore the relationship between daily physical activity time and maximal fat oxidation in patients with cardiovascular disease.Methods This was a cross-sectional study.Cardiovascular disease patients who underwent cardiopulmonary exercise test(CPET)in Beijing Hospital from April of 2021 to December of 2022 were included.Clinical data and CPET data were collected while International Physical Activity Questionnaire was used to investigate the physical activity time of the patients.Oxygen uptake(VO2),VO2 accounted for predicted maximum value,oxygen pulse and VE/VCO2 on the time of maximal fat oxidation,anaerobic threshold and peak time were recorded.Physical activity time was divided into four groups according to four quadrants(P0-P25,>P25-P50,>P50-P75,>P75-P100)and CPET parameters were compared between those groups.CPET parameters were also compared between four Weber cardiac function groups(Weber A,B,C and D grade).Pearson simple correlation analysis was used to explore the relationship between daily physical activity time and maximal fat oxidation.Results A total of 180 patients were finally enrolled.There were 100 male and 80 female patients with an average age of(62.76±10.98)years old.Maximal fat oxidation and FATmax(VO2 at the time of maximal fat oxidation accounted for predicted maximum V02)did not show difference in four physical activity time groups.VE/VCO2 on maximal fat oxidation(P=0.021),VE/VCO2 on aerobic threshold(P=0.006)and peak VE/VCO2(P<0.001)showed significant difference in four physical activity time groups.Maximal fat oxidation(P<0.001),FATmax(P<0.001),oxygen pulse on maximal fat oxidation(P<0.001),and VE/VCO2 on maximal fat oxidation(P=0.002)were significant different between four heart function groups according Weber classification.Pearson correlation analysis showed that physical activity time was not significant related with maximal fat oxidation(P=0.658),FATmax(P=0.754)and VE/VCO2 on maximal fat oxidation(P=0.054),but significant related with VE/VCO2 on aerobic threshold(P=0.033)and VE/VCO2 on peak time(P=0.010).Conclusions In cardiovascular disease patients,daily physical activity time may not be correlated with maximal fat oxidation and FATmax,but may be correlated with VE/VCO2.Maximal fat oxidation may exist difference in the patients with different cardiac function.The worse cardiac function,the lower maximal fat oxidation and earlier occurrence time.
