摘要
我国是世界上食管鳞状细胞癌(esophageal squamous cell carcinoma,ESCC)发病率最高的国家.ESCC的早期诊断和治疗一直是临床研究的热点.浅表食管鳞状细胞癌(superficial esophageal squamous cell carcinoma,SESCC)属于较为早期的ESCC,临床上相当比例的SESCC可以行内镜下切除治疗.但在进行内镜治疗前,准确判断病变浸润深度,识别肿瘤边缘,评估淋巴结及远隔转移情况,进行精准分期是至关重要的,也是决定内镜治疗的可行性和选择治疗方式的关键;而ESCC浸润深度与淋巴结转移、分期及预后密切相关.其中内镜是临床上评估ESCC浸润深度的重要手段,不同类型内镜在诊断SESCC时各有优劣.本文综述SESCC浸润深度的内镜诊断进展,以期为临床研究提供参考.
Abstract
China is one of the countries with the highest incidence of esophageal squamous cell carcinoma(ESCC)all around the world.Early diagnosis and treatment of ESCC has always been a hot topic in clinical researches.Superficial esophageal squamous cell carcinoma(SESCC)is an early stage of ESCC,and a considerable proportion of SESCC can be treated with endoscopic resection.Before endoscopic treatment,it is of vital importance to predict accurately the depth of lesion invasion,identify the margin of lesion,evaluate metastasis of lymph nodes and distant organs,and ascertain clinical stage.The depth of ESCC is crucial to determine the feasibility of endoscopic treatment and the selection of different treatment modalities,which is closely related to lymph node metastasis,stage and prognosis.Endoscopic devices are important for evaluation the infiltration depth of ESCC in clinical practice.Different endoscopes have their own advantages and disadvantages in the diagnosis of SESCC.We reviewed the progress in endoscopic diagnosis of the depth of invasion of SESCC for providing reference for clinical researches.
