摘要
丘脑底核脑深部电刺激(subthalamic nucleus deep brain stimulation,STN-DBS)是治疗帕金森病(Parkinson's disease,PD)运动症状的有效方法.近年来,越来越多的研究关注其对情感和认知功能的影响.本文综述了 STN-DBS治疗PD患者情感和认知功能的短期和长期影响.短期研究显示,STN-DBS可显著改善患者的焦虑和抑郁症状,同时对认知功能影响较小.然而,长期随访研究表明,情感改善效果可能随着时间的推移减弱,认知功能可能出现显著下降.此外,丘脑底核不同亚区的刺激对情感和认知功能的影响存在差异.前外侧区域的刺激在情感改善方面效果更好,而腹侧区域的刺激则对认知功能保护作用更强.本文强调了在实施STN-DBS治疗时,应持续监测和管理患者的心理和认知功能,并考虑不同亚区刺激的精准定位以提高疗效.
Abstract
Subthalamic nucleus deep brain stimulation(STN-DBS)is an effective method for treating motor symptoms in Parkinson's disease(PD).In recent years,increasing research has focused on its impact on emotional and cognitive functions.This paper reviews the short-term and long-term effects of STN-DBS treatment on the emotional and cognitive functions of patients with Parkinson's disease.Short-term studies show that STN-DBS can significantly improve patients'anxiety and depression symptoms while having a minimal impact on cognitive function.However,long-term follow-up studies indicate that the emotional improvements may diminish over time,and cognitive functions may experience significant decline.Additionally,the impact of stimulating different subregions of the subthalamic nucleus on emotional and cognitive functions varies.Stimulation in the anterolateral region has better effects on emotional improvement,while stimulation in the ventral region offers better cognitive protection.This paper emphasizes the importance of continuous monitoring and management of patients'psychological and cognitive functions during STN-DBS treatment.It also emphasizes the need to consider precise targeting of different subregions to enhance therapeutic efficacy.
