摘要

目的 探讨在规范化、标准化电子鼻咽喉镜检查中盐酸利多卡因胶浆跟盐酸丁卡因的麻醉效果.方法 将我科门诊958例行电子喉镜检查的按就诊顺序奇数患者归入利多卡因胶浆表麻组(L组),偶数患者归入丁卡因表麻组(D组),每组479例.分别从两组中白光内镜发现病变通过窄带成像(NBI)内镜观察病变表面的微血管患者115例(L1组)和124例(D1组),比较L、D两组及L1、D1两组患者喉镜检查过程中完成喉镜检查所需时间及总体感受评分.结果 完成电子鼻镜检查所需时间对比如下:L、D两组比较差异有统计学意义(P=0.000),Ll、D1两组比较差异无统计学意义(P=0.059);L、L1两组患者内镜检查总体感受评分低于D、D1组;它们比较差异均有统计学意义(P=0.000).结论 盐酸利多卡因胶浆在电子鼻咽喉镜表面麻醉中具有起效快、麻醉风险低、患者咽喉舒适度好等特点,更利于电子喉镜发现病变并通过NBI内镜观察病变表面的微血管形态,在规范化、标准化的电子喉镜中值得推广运用.

Abstract

Objective To investigate the anesthetic effect of lidocaine hydrochloride mucilage and tetracaine hydrochloride in standardized and standardized Electronic laryngoscope examination.Methods In this study,958 patients who were examined by electronic laryngoscope were assigned to Lidocaine mucilage group(Group L)and Tetracaine mucilage group(Group D),479 patients in each group.The lesions were found by white light endoscopy in 115 patients(Group L 1)and 124 patients(Group D 1)with narrow band imaging(NBI),the time and overall feeling score of patients in L,D group and L 1,D 1 group during Laryngoscopy were compared.Results The time to complete the electronic nose examination was as follows:There was significant difference between 1 group and D Group(p=0.000),but no significant difference between 11 and D 1 Group(p=0.059);The general feeling score of endoscopy in a group and L 1 group was lower than that inD group and D 1 Group(p=0.000).Conclusion Lidocaine hydrochloride mucilage in surface anesthesia of Electronic laryngoscope has the characteristics of quick onset,low anesthesia risk,good throat comfort,etc.,it is more advantageous to find the lesion by electronic laryngoscope and observe the microvascular morphology on the surface of the lesion by NBI endoscope.
