扁桃体摘除术行瑞芬太尼-丙泊酚靶控输注的麻醉效果

摘要
目的 研究瑞芬太尼-丙泊酚靶控输注用于扁桃体摘除术中的效果.方法 数据取自本院2021年1月-2022年6月收治的60例扁桃体摘除术患儿,"双盲法"分基础组(丙泊酚+芬太尼诱导麻醉,吸入七氟醚维持麻醉,n=30)、研讨组(瑞芬太尼-丙泊酚靶控输注,n=30),两组生命体征、应激反应、术后恢复情况及不良反应比较分析.结果 麻醉前比较两组生命体征、应激反应无差异,P＞0.05;麻醉后与基础组比较,研讨组DBP、SBP及HR指标更低;CRP指标更低;自主呼吸恢复、睁眼、拔管及离开恢复室时间更短;研讨组不良反应发生率(3.33％)低于基础组(20.00％),x2=4.043,p=0.044,P＜0.05(具有统计学意义).结论 扁桃体摘除术患儿行瑞芬太尼-丙泊酚靶控输注可稳定生命体征、减轻应激反应,促进自主呼吸恢复,缩短恢复时间、减少不良反应发生,值得推崇.
Abstract
Objective To study the effect of target-controlled infusion of remifentanil-propofol in tonsillectomy.Methods Data were collected from 60 children undergoing tonsillectomy in our hospital from January 2021 to June 2022.The"double-blind"method was divided into the basic group(propofol+fentanyl for induction anesthesia,inhaled sevoflurane for maintenance Anesthesia,w=30),the study group(target-controlled infusion of remifentanil-propofol,n=30),the two groups'vital signs,stress response,postoperative recovery and adverse reactions were compared and analyzed.Results Before anesthesia,there was no difference in vital signs and stress response between the two groups,P＞0.05;after anesthesia,compared with the basic group,the DBP,SBP and HR indexes of the research group were lower;CRP indexes were lower;The time of extubation and leaving the recovery room was shorter;the incidence of adverse reactions in the study group(3.33％)was lower than that in the basic group(20.00％),x2=4.043,p=0.044,P＜0.05(statistically significant).Conclusion Target-controlled infusion of remifentanil-propofol in children undergoing tonsillectomy can stabilize vital signs,reduce stress response,promote the recovery of spontaneous breathing,shorten the recovery time,and reduce the occurrence of adverse reactions,which is worthy of praise.
