术前口腔洁治对慢性扁桃体炎术后感染发生率的影响

摘要
目的 分析在术前行口腔洁治对于控制慢性扁桃体炎患者术后感染发生率的作用.方法 选取2019年1月～2019年12月遵义市第一人民医院耳鼻咽喉科行低温等离子射频消融手术治疗的慢性扁桃体炎患者120例,随机选取同意及能够配合洁治的患儿纳入洁治组,其他的为非洁治组,每组各60例.结果 在接受手术前所有患者的相关炎性指标水平均未呈现出明显差异,在手术后,参与术前口腔洁治的患者各项炎性指标的水平均明显低于对照组,存在显著差异,P＜0.05;观察组术后1d平均疼痛评分5.26±0.15分,到术后1周下降到1.04±0.02分,对照组术后1d平均评分6.38±0.10分,到术后1周下降到3.51±0.08分,组间对比差异显著,P＜0.05;在时间性指标对比,观察组患者白膜生长时间2.62±0.05d,性质及脱落时间6.52±1.26d,局部肿胀消退时间3.62±0.25d,对照组患者白膜生长时间3.98±0.01d,性质及脱落时间10.51±1.08d,局部肿胀消退时间6.58±0.14d,组间对比差异显著,P＜0.05;观察组中有例患者表示不满意,满意度为98.33％,对照组中有10例患者表示不满意,总满意度为83.33％,组间对比差异显著,P＜0.05.结论 口腔洁治在慢性扁桃体炎术前的应用,可降低患者术后感染风险,可促进术后快速康复,值得推广.
Abstract
Objective To analyze the effect of oral cleaning before operation on controlling the incidence of postoperative infection in patients with chronic tonsillitis.Methods 120 patients with chronic tonsillitis who underwent low temperature plasma radiofrequency ablation in otorhinolaryngology department of Zunyi first people's hospital from January 2019 to December 2019 were selected.children who agreed and could cooperate with the cleaning were randomly selected and included in the cleaning group,while the others were non-cleaning group,60 cases in each group Result Before surgery,the levels of relevant inflammatory indicators of all patients did not show significant differences.After surgery,the levels of various inflammatory indicators of patients participating in preoperative oral cleansing were significantly lower than those of the control group,which had significant differences.P ≤0.05;the average pain score in the observation group at 1d after surgery was 5.26±0.15,and it dropped to 1.04±0.02 at 1 week after surgery;in the control group,the average score at 1d after surgery was 6.38±0.10,and it dropped to 3.51±0.08 at 1 week after Points,the difference between groups is significant,P≤0.05;In the time index comparison,the growth time of white membrane in the observation group was 2.62±0.05d,the nature and fall off time was 6.52±1.26d,the local swelling subsided time was 3.62±0.25D,the growth time of white membrane in the control group was 3.98± 0.01d,the nature and fall off time was 10.51±1.08d,the local swelling subsided time was 6.58±0.14d,the difference between the groups was significant,P ≤ 0.05;some patients in the observation group were not satisfied.The satisfaction rate was 98.33％,10 patients in the control group were dissatisfied,the total satisfaction rate was 83.33％,the difference between the groups was significant,P ≤ 0.05.Conclusion The application of oral cleansing before the operation of chronic tonsillitis can reduce the risk of postoperative infection and promote the rapid recovery after the operation,which is worth popularizing.
