The diagnostic value of shear wave combined with strain ultrasound elastography in hemorrhagic ovarian cyst and endometriosis

目的 探讨剪切波联合应变式超声弹性成像(strain ultrasonic elastography,SE)技术在出血性卵巢囊肿与子宫内膜异位症中的诊断价值及指导意义.方法 选取承德市中心医院2019 年1 月至2021 年5 月盆腔肿物患者126 例作为研究组,另选取同期健康体检女性 63 例作为对照组,均行剪切波超声弹性成像(shear wave ultrasonic elastography,SWE)与SE检查,比较两组剪切波速度(SWV),弹性模量平均值(Emean)、最大值(Emax)、最小值(Emin),应变率比值(SR).将研究组根据手术病理检查结果进行划分,子宫内膜异位症(60例)为内膜异位亚组,出血性卵巢囊肿(66 例)为卵巢囊肿亚组,分析SWE联合SE检查诊断出血性卵巢囊肿与子宫内膜异位症的价值.结果 研究组SWV、Emean、Emax、Emin、SR高于对照组(P<0.05);内膜异位亚组SWV、Emean、Emax、Emin、SR高于卵巢囊肿亚组(P<0.05);SWE影像学表现:与出血性卵巢囊肿相比,子宫内膜异位症SWV较快,弹性值偏高;SE影像学表现:子宫内膜异位症应变彩色编码主要呈红色或橙红色,应变类型主要为3 型,SR值偏高;出血性卵巢囊肿应变彩色编码主要呈绿色或黄绿色,少数呈蓝色或蓝绿色与红色或橙红色,应变类型集中在2 型,SR相对较低;Pearson相关性分析,SR与SWV、Emean、Emax、Emin呈正相关(P<0.05);ROC曲线分析,SWV、Emean、Emax、Emin、SR联合诊断的AUC为 0.924(95%CI:0.863-0.964),敏感度为90.00%,特异度为86.36%,明显优于各参数单独诊断.结论 SWE联合SE检查在出血性卵巢囊肿与子宫内膜异位中具有较高诊断价值,能为临床诊治提供可靠的指导信息.
Objective To investigate the diagnostic value and guiding significance of shear wave combined with strain ultrasound elastography(SE)in hemorrhagic ovarian cyst and endometriosis.Methods A total of 126 patients with pelvic masses in Chengde Central Hospital from January 2019 to May 2021 were selected as the research group,and 63 healthy women who underwent physical examination during the same period were selected as the control group.All patients underwent shear wave ultrasound elastography(SWE)and SE examination,compared the shear wave velocity(SWV),the mean value(Emean),the maximum value(Emax)and the minimum value(Emin)of elastic modulus and the strain rate ratio(SR)between the two groups.According to the pathological examination results,the research group were divided into the endometriosis subgroup(60 cases),hemorrhagic ovarian cyst(66 cases)as the ovarian cyst subgroup,and analyzed the value of SWE combined with SE in the diagnosis of hemorrhagic ovarian cyst and endometriosis.Results SWV,Emean,Emax,Emin and SR in the research group were higher than those in the control group(P<0.05);SWV,Emean,Emax,Emin and SR in the endometriosis subgroup were higher than those in the ovarian cyst subgroup(P<0.05);SWE imaging manifestations:compared with hemorrhagic ovarian cysts,endometriosis had a faster SWV and higher elasticity value;SE imaging manifestations:The color coding of endometriosis strains was mainly red or orange-red,and the strain type mainly type 3,with high SR value;color coding of hemorrhagic ovarian cysts was mainly green or yellow-green,and a few were blue or blue-green and red or orange-red,strain types are concentrated in type 2,and SR was relatively low;Pearson correlation analysis showed that SR was positively correlated with SWV,Emean,Emax and Emin(P<0.05);according to ROC curve analysis,the AUC of the combined diagnosis of SWV,Emean,Emax,Emin and SR was 0.924(95% CI:0.863-0.964),with a sensitivity of 90.00% and a specificity of 86.36%,which was significantly better than that of each parameter alone.Conclusion SWE combined with SE examination has high diagnostic value in hemorrhagic ovarian cyst and endometriosis,and can provide reliable guidance information for clinical diagnosis and treatment.
