血清因子对子宫内膜异位症患者发生不孕症的预测价值分析

摘要
目的 探究血清因子对子宫内膜异位症(endometriosis,EMS)患者发生不孕症的预测价值.方法 将2020 年8 月至2021 年9 月在曹妃甸区医院行腹腔镜手术经病理确诊为EMS的患者110 例纳入研究,按是否合并不孕症分为EMS组(n =60)和EMS不孕组(n =50),将同期健康体检者作为对照组(n =50).比较3组患者血清因子[白细胞介素-6(IL-6)、白细胞介素-37(IL-37)、肿瘤坏死因子(TNF-α)、转化生长因子-β(TGF-β)]的水平,用Pearson法分析血清因子间的相关性,用受试者工作特征曲线(ROC)分析血清因子对EMS不孕症的预测价值.结果 EMS组、EMS不孕组的血清IL-6、IL-37、TNF-α、TGF-β水平均高于对照组,且EMS不孕组高于EMS组,差异有统计学意义(P<0.05).Pearson分析显示,血清IL-6、IL-37、TNF-α、TGF-β均呈正相关性(P<0.05).ROC曲线分析显示,血清IL-6、IL-37、TNF-α、TGF-β预测 EMS 与 EMS 不孕症的 AUC 分别为 0.715、0.687、0.728、0.716,联合检测的灵敏度(97.8%)、特异度(75.0%)均高于单独检测(P<0.05).结论 血清IL-6、IL-37、TNF-α、TGF-β对EMS与EMS不孕症均有较高预测价值,且血清因子间呈正相关.
Abstract
Objective To explore the predictive value of serum factors for infertility in patients with endometriosis(EMS).Methods A total of 110 patients with EMS who underwent laparoscopic surgery and pathologically diagnosed in Caofeidian District Hospital from August 2020 to September 2021 were included in the study.They were divided into EMS group(n =60)and EMS infertility group(n =50)according to whether they were complicated with infertility,and healthy subjects in the same period were taken as control group(n =50).The levels of serum factors including interleukin-6(IL-6),interleukin-37(IL-37),tumor necrosis factor-α(TNF-α),transforming growth factor-β(TGF-β)in 3 groups were compared,and the correlation between serum factors was analyzed by Pearson method.The predictive value of serum factors for EMS infertility was analyzed by receiver operating characteristic curve(ROC).Results The serum levels of IL-6,IL-37,TNF-α and TGF-β in EMS group and EMS infertility group were higher than those in control group,and those in EMS infertility group were higher than those in EMS group,the differences were statistically significant(P<0.05).Pearson analysis showed that serum IL-6,IL-37,TNF-α and TGF-β were positively correlated(P<0.05).ROC curve analysis showed that the AUC of serum IL-6,IL-37,TNF-α and TGF-β in predicting EMS and EMS infertility were 0.715,0.687,0.728 and 0.716,respectively.The sensitivity(97.8%)and specificity(75.0%)of combined detection were higher than those of single detection(P<0.05).Conclusion Serum IL-6,IL-37,TNF-α and TGF-β showed high predictive value for EMS and EMS infertility,and the serum factors were positively correlated.
