目的 研究Gremlin-2基因rs4454537基因多态性及血清25-羟维生素D3[25(OH)D3]水平异常对绝经后妇女骨质疏松的影响.方法 选取邢台市人民医院2020年2月至2023年10月收治的绝经后骨质疏松患者160例为研究组,并选取同期健康体检的绝经女性160例为对照组,比较两组妇女骨密度及25(OH)D3水平,并分析两者相关性.采用TaqMan探针法检测患者血液中Gremlin-2基因rs4454537位点基因多态性,比较两组基因分布情况、不同基因型患者血清25(OH)D3水平以及骨密度差异,并分析绝经后妇女骨质疏松的影响因素.结果 研究组的25(OH)D3表达水平以及正位骨密度(BMD)、T值均低于对照组(P<0.05).Pearson 法分析显示 25(OH)D3 表达水平与 BMD 呈正相关性(r=0.638,P<0.05).Gremlin-2 基因 rs4454537位点存在3种基因型:TT、TC和CC,研究组的T等位基因、TT基因型频率低于对照组(x2=19.643,P<0.001,x2=18.321,P<0.001).不同基因型患者骨密度、25(OH)D3水平差异有统计学意义,其中TT型患者的骨密度、25(OH)D3水平明显高于TC、CC基因型患者(均P<0.05).Logistic回归分析结果显示,Gremlin-2基因rs4454537位点基因型、25(OH)D3水平是绝经后骨质疏松的独立影响因素(P<0.01).结论 Gremlin-2基因rs4454537位点基因多态性以及血清25(OH)D3水平与绝经后妇女骨质疏松密切相关,其中TT基因型患者血清25(OH)D3水平处于较高水平,可能降低骨质疏松发生风险.
Objective To study the effects of rs4454537 polymorphism of Gremlin-2 gene and abnormal level of 25-hydroxyvitamin D3[25(OH)D3]on osteoporosis in postmenopausal women.Methods 160 postmenopausal osteoporosis patients admitted to Xingtai People's Hospital from February 2020 to October 2023 were selected as the study group,and 160 healthy menopausal women who underwent physical examination during the same period were selected as the control group.Bone mineral density(BMD)and 25(OH)D3 levels of the two groups were compared,and the correlation of which was analyzed.TaqMan probe method was used to detect the polymorphism of Gremlin-2 gene rs4454537 in the blood of patients,and the gene distribution,serum 25(OH)D3 level and BMD of patients with different genotypes were compared,and the influencing factors of osteoporosis in postmenopausal women were analyzed.Results The expression level of 25(OH)D3,BMD and T value in the study group were lower than those in the control group(P<0.05).The expression level of 25(OH)D3 was positively correlated with BMD by Pearson analysis(r=0.638,P<0.05).There were three genotypes at the rs4454537 locus of Gremlin-2 gene:TT,TC and CC.The frequencies of T allele and TT genotype in the study group were lower than those in the control group(x2=19.643,P<0.001,x2=18.321,P<0.001).BMD and 25(OH)D3 levels in patients with different genotypes were significantly different,and BMD and 25(OH)D3 levels in patients with TT genotype were significantly higher than those in patients with TC and CC genotype(P<0.05).Logistic regression analysis showed that rs4454537 genotype of Gremlin-2 gene and 25(OH)D3 level were independent influencing factors for postmenopausal osteoporosis(P<0.01).Conclusions The rs4454537 locus polymorphism of Gremlin-2 gene and serum 25(OH)D3 level are closely related to osteoporosis in postmenopausal women,and TT genotype patients have a high level of serum 25(OH)D3 level,which may decrease the risk of osteoporosis.
