Diagnostic and prognostic value of serum AMH in PCOS patients of different ages
Objective To analyze the diagnostic and prognostic value of serum anti-Müllerian hormone(AMH)level in patients with polycystic ovary syndrome(PCOS)at different ages.Methods From June 2022 to February 2023,157 PCOS patients aged 20~40 years old who were treated in Jinjiang Maternal and Child Health Hospital of Chengdu were selected as the observation group,and 135 pre-pregnancy patients of the same age and with regular menstruation who came to the hospital for physical examination during the same period were selected as the control group.According to the age,the observation group and the control group were divided into two subgroups:20~<30-years-old group and 30~40-years-old group,and the age and AMH level of each group were compared.Receiver operating characteris(ROC)curve analysis was performed using GraphPad Prism to evaluate the efficacy of AMH in the diagnosis of PCOS in different age groups.After half a year of intervention in the observation group,all patients were followed up,and those with incomplete data were excluded and then divided into good prognosis group(36 cases)and poor prognosis group(11 cases)according to ultrasound diagnosis,and the general condition and AMH level of the two groups were compared,and the prognostic value of AMH on PCOS was analyzed.Results The AMH level in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The AMH levels of different age subgroups in the observation group were higher than those in the control group,and the AMH levels in the 20~<30-year-old group in the control group were higher than those in the 30~40-year-old group,and the differences were statistically significant(P<0.05).There was no significant difference in AMH levels among different age subgroups in the observation group(P>0.05).The AUC of ROC curves for AMH diagnosis of PCOS in the 20~<30-year-old group and 30~40-year-old group were 0.8575 and 0.9257,respectively,and the optimal cut-off values were 6.22 ng/mL and 4.57 ng/mL,respectively.Spearman correlation analysis showed that there was no significant correlation between AMH and prognosis of PCOS(P>0.05).Conclusion The level of AMH in PCOS patients is higher than that in the general population,and AMH has high sensitivity and specificity in the diagnosis of PCOS,and its diagnostic efficacy is different in different ages,with different optimal cut-off values,but AMH has no obvious significance in the prognostic assessment of PCOS.
