Analysis of trajectory and influencing factors of fear of disease progression in patients with endovascular radiotherapy for cervical cancer
Objective To investigate the trajectory of fear of disease progression and analysis of influencing factors in patients with endovascular radiotherapy for cervical cancer.Methods A total of181 patients with endovascular radiotherapy for cervical cancer treated in Nanjing Chest Hospital from May 2020 to March 2021 were selected. Baseline investigation was conducted using the general data questionnaire,fear of progression questionnaire-short form( FoP-Q-SF),family PAGAR index( PAGAR) and self-perceived burden scale. After the investigation,patients were followed up before radiotherapy,at the end of radiotherapy,1 month after radiotherapy and 3 months after radiotherapy(T0~T3). Differences in the fear of progression of cervical cancer patients at different time periods were analyzed,and subgroups of fear of progression were identified by the latent category model. Finally,the decision tree model was used to analyze the factors affecting the trajectory of fear change in disease progression. Results The scores of the patients with intralucal radiotherapy for cervical cancer at different follow-up time points were 36.41±7.35 points in T0,37.83±9.16 points in T1,40.62±10.28 points in T2,42.41±10.35 points in T3,and there were significant differences in the scores at different time points ( P<0.01). According to the latent category model,two fear trajectories of disease progression were identified in the medium disease fear-fast rising group (17.68%) and the low disease fear-slow rising group (82.32%). The results of decision tree analysis showed that age,education level,stage of cervical cancer,monthly family income,good family function and self-perceived burden could all predict the trajectory of fear of disease progression in patients with endovascular radiotherapy for cervical cancer,among which good family function was the most important influencing factor.Conclusion The level of fear of disease progression in patients with intracavitary radiotherapy for cervical cancer is generally on the rise,and there is a certain group heterogeneity in the change trajectory of fear of disease progression,and family function is its core influencing factor. Based on the above factors,medical personnel can identify high-risk patients with disease fear and rapid rise in the early stage and timely formulate intervention programs with family function as the core,this in turn helps reduce the patient's fear of disease progression.
