Pathological characteristics of early young cervical cancer patients and prognosis analysis after transabdominal radical hysterectomy
Objective To explore the pathological characteristics of early young cervical cancer patients and the influencing factors of prognosis after transabdominal radical hysterectomy. Methods A total of 92 early young(age≤35 years old)cervical cancer patients as the study group, and 93 early cervical cancer patients(age>35 years old)as the control group who were admitted to the Shashi District Maternal and Child Health Care Hospital in Jingzhou from January 2015 to January 2017 were included in the study, respectively. The differences in pathological characteristics between the two groups of patients were compared, the prognosis of transabdominal radical hysterectomy was recorded, and the relationship between pathological characteristics and the prognosis of early young cervical cancer patients was analyzed. Results The proportion of squamous cell carcinoma patients in the study group was significantly lower than that in the control group, and the positive rate of human papilloma virus(HPV)was significantly higher than that in the control group(P<0.05).There were 19 cases of recurrence in the study group, with a recurrence rate of 20.65%.The survival time was 32-71 months, with a median survival time of 52 months and a 5-year survival rate of 84.78%(85/92).Univariate analysis showed that tumor diameter, infiltration of lymphatic vessel spaces, and depth of interstitial infiltration were factors that affected the recurrence and 5-year survival rate of patients(P<0.05).COX multivariate analysis showed that positive infiltration of lymphatic vessel spaces and depth of interstitial infiltration≥1/2 were independent risk factors for the recurrence and poor prognosis of early young cervical cancer(P<0.05).Conclusion Early young cervical cancer patients have the characteristics of a low proportion of squamous cell carcinoma and a high incidence of HPV infection.Clinicopathological factors such as infiltration of lymphatic vessel spaces and depth of interstitial infiltration are factors that affect the recurrence and survival time of early young cervical cancer patients after transabdominal radical hysterectomy and are worthy of clinical attention.
