Analysis of pathological type and serum anti-PLA2R antibody level of kidney biopsy in the elderly
Objective To analyze the pathological types of renal biopsy and serum anti-M-type phospholipase A2 receptor(PLA2R)antibody levels in elderly nephrotic syndrome.Methods Retrospectively analyzed the clinical data of 157 elderly renal biopsy patients admitted to Hechi People's Hospital from June 2013 to September 2021,described the pathological types of the patients'kidneys,compared the pathological changes before and after 4 years,and analyzed the serum anti-PLA2R antibody levels.Results Among the 157 elderly patients with renal biopsy pathological types,103 cases(65.61%)of membranous nephropathy,11 cases(7.01%)of ANCA-related vasculitis,9 cases(5.73%)of minimal change glomerulopathy,IgA 5 cases of nephropathy(3.18%),4 cases of focal segmental glomerulosclerosis(2.55%),4 cases of diabetic nephropathy(2.55%),4 cases of lupus nephritis(2.55%),4 cases of tubular interstitial disease Cases(2.55%),the other 13 cases(8.28%).By comparing the changes of the pathological spectrum before and after 4 years,it was found that the proportion of membranous nephropathy was 64.71%66.67%respectively,and the proportion of ANCA-related vasculitis increased from 4.17%to 9.41%.The sensitivity of serum anti-PLA2R antibody to diagnose membranous nephropathy was 73.33%(11/15),the specificity was 0,the positive predictive value was 100.00%(11/11),and the accuracy was 73.33%(11/15).Conclusion The common pathological type of renal biopsy in the elderly is mainly membranous nephropathy,followed by ANCA-associated vasculitis,and the latter has an upward trend.Serum anti-PLA2R antibody has a particular value in the diagnosis of elderly membranous nephropathy.
