股骨颈动力交叉钉系统治疗股骨颈骨折的临床效果

目的 探讨股骨颈动力交叉钉系统治疗股骨颈骨折的临床效果。方法 选取 2020 年 8 月至 2022 年7 月厦门市第五医院骨科收治股骨颈骨折患者 60 例为研究对象,根据随机数表法分为对照组与观察组,每组各 30 例,对照组接受 3 根空心螺钉内固定治疗,观察组采用股骨颈动力交叉钉系统固定治疗,比较两组复位质量与关节功能。结果 观察组患者术中透视次数(11。60±2。04)次显著低于对照组的(20。45±3。70)次,术中出血量(12。25±2。26)ml低于对照组的(18。79±3。69)ml,而手术时间为(48。02±4。13)min短于对照组的(67。14±4。86)min,首次负重时间(11。23±2。08)周短于对照组的(15。37±2。46)周,骨折愈合时间为(4。01±0。24)个月短于对照组的(4。89±0。31)个月(P<0。05);观察组复位质量Ⅰ级占比高于对照组,而Ⅱ、Ⅲ、Ⅳ级占比低于对照组(P<0。05);术后 6 个月观察组髋关节功能评分为(72。40±4。85)分,与对照组的(70。82±5。13)分比较,差异无统计学意义(P>0。05)。结论 股骨颈动力交叉钉系统固定术用于股骨颈骨折中创伤较小,术中透视次数少,手术时间、首次负重时间、愈合时间短,可提高复位质量,并且对髋关节功能无影响,可作为首选固定方案。
Clinical effect of femoral neck system for the treatment of femoral neck fractures
Objective To explore the clinical effect of the femoral neck system for the treatment of femoral neck fractures.Methods A total of 60 patients with femoral neck fractures admitted to the Department of Orthopedics of Xiamen Fifth Hospital from August 2020 to July 2022 were selected as the study subjects.They were divided into a control group and an observation group by the random number table method,with 30 cases in each group.The control group received 3 hollow screws for internal fixation,while the observation group was fixed with the femoral neck system.The reduction quality and joint function of the two groups were compared.Results The observation group had significantly lower intraoperative fluoroscopy frequency(11.60±2.04)compared to the control group(20.45±3.70),less intraoperative bleeding volume(12.25±2.26)ml compared to the control group(18.79±3.69)ml,shorter surgical time(48.02±4.13)minutes compared to the control group(67.14±4.86)minutes,shorter initial weight-bearing time(11.23±2.08)weeks compared to the control group(15.37±2.46)weeks,and shorter fracture healing time(4.01±0.24)months compared to the control group(4.89±0.31)months(P<0.05).The proportion of type Ⅰ reduction quality in the observation group was higher than that in the control group.The proportion of type Ⅱ,Ⅲ and Ⅳ reduction quality was lower than that in the control group(P<0.05).After 6 months of surgery,the score of the hip joint function between the observation group(72.40±4.85)points and the control group(70.82±5.13)points was compared,without significant difference(P>0.05).Conclusion The femoral neck system fixation technique for femoral neck fractures has less trauma,fewer intraoperative fluoroscopy times,and shorter surgical time,first weight-bearing time,and healing time,which can improve the reduction quality and has no impact on hip joint function,making it the preferred fixation scheme.
