Analysis on the clinical characteristics of septicemia in premature neonates with different gestational ages
Objective To analyze the clinical characteristics of septicemia in 175 premature neonates with different gestational ages. Methods A total of 175 premature neonates diagnosed with neonatal septicemia in the neonatal ward of the First Affiliated Hospital of Guangxi Medical University from January 2016 to November 2020 were analyzed retrospectively.According to the gestational age at birth, they were divided into the ultra-premature infants group(n=21),the extreme-premature infants group(n=78)and the mid-late premature infants group(n=76).The clinical data of the three groups of affected children were compared and the results were analyzed. Results The probability of invasive mechanical ventilation, invasive operation before septicemia, fasting status before septicemia and the incidence of using antibiotics before septicemia in the ultra-premature infants group were higher than those in the extreme-premature infants group and the mid-late premature infants group. There were statistically significant differences in clinical manifestations manifested as fever and dyspnea between the mid-late premature infants group and the other two groups(P<0.05).The incidence of intracranial hemorrhage and pulmonary hemorrhage in the mid-late premature infants group was lower than that in the other two groups, the incidence of purulent meningitis in the extreme-premature infants group was higher than that in the other two groups, and the incidence of shock in the ultra-premature infants group was higher than that in the other two groups, with statistically significant differences(P<0.05).The utilization rate of vasoactive drugs in the mid-late premature infants group was lower than that in the other two groups, the average course of antibiotics was shorter than that in the other two groups, and the mechanical ventilation time in the treatment of the ultra-premature infants group was longer than that in the other two groups, with statistically significant differences(P<0.05).Conclusion There are differences in clinical characteristics, disease prognosis, complications and treatment of neonatal septicemia in the premature infants with different gestational ages.It is helpful to guide clinical treatment to be familiar with different clinical characteristics.
