Exploring the impact of nursing intervention on the readmission of chronic heart failure patients with relapse
Objective To explore the impact of nursing intervention on the readmission of chronic heart failure patients with relapse. Methods A total of 180 patients diagnosed with chronic heart failure admitted to Quanzhou First Hospital(from March 2021 to February 2022)were selected as the study subjects. They were divided into two groups of 90 patients each using a random number table method. One group received routine nursing intervention as the control group, while the other group received comprehensive-targeted health continuity nursing intervention as the study group. The scores of two groups of patients on their mastery of knowledge about chronic heart failure-related diseases, and compliance with nursing guidance and medical orders before and after nursing care was compared.Followed up for 1 year to calculate the recurrence readmission rate and patient satisfaction with nursing care. Results After nursing, the scores of disease knowledge, medication knowledge, diet knowledge, chronic heart failure-induced complications and coping mode knowledge, cardiac rehabilitation knowledge and the scores of compliance with nursing instructions and medical orders such as diet, medication, exercise health, life health and regular review were improved in both groups compared with those before nursing, and the scores of the study group were higher than those of the control group. The difference was statistically significant(P<0.05).The recurrence and readmission rates of patients in the study group after one year of follow-up were lower than those in the control group, while the overall satisfaction rate of patients with nursing care was higher than that in the control group, with statistically significant differences(P<0.05).Conclusion In patients with chronic heart failure, implementing comprehensive and targeted health continuity nursing interventions can significantly enhance their ability to master disease-related knowledge, improve their compliance with nursing guidance and medical orders, significantly reduce the rates of relapse and readmission, gain consistent patient satisfaction, and achieve ideal results.
