Research on the construction and application of ERAS-CNP in the perioperative period of ERCP in the treatment of calculus of common bile duct
Objective To construct the clinical nursing pathway of enhanced recovery after surgery(ERAS-CNP)for endoscopic retrograde cholangio-pancreatography(ERCP)in the treatment of calculus of common bile duct during perioperative period,so as to investigate its clinical application efficacy.Methods A total of 120 patients with calculus of common bile duct treated by ERCP admitted to the First General Hospital of Fuzhou City from June 2022 to June 2023 were selected and divided into the control group(n=60)and the experimental group(n=60)according to the random number table method.The control group was treated with perioperative nursing conventional nursing mode,while the experimental group was strictly followed by ERAS-CNP to nursing.The surgical therapeutic efficacy index,benefit index,clinical related index,psychological emotion,comfort,pain control situation and nursing work satisfaction were compared between the two groups of patients.Results The first time of eating,the first time of getting out of bed,hospitalization time and hospitalization expenses of patients in the experimental group were all significantly superior to those in the control group,with statistically significant differences(P<0.05).After operation,there was no statistically significant difference in amylase levels between the two groups of patients(P>0.05),while the levels of white blood cell count(WBC)and C-reactive protein(CRP)of patients in the experimental group were lower than those in the control group,with statistically significant differences(P<0.05).There were no statistically significant differences in the incidences of post-ERCP pancreatitis(PEP),secondary biliary tract infection and gastrointestinal hemorrhage between the two groups(P>0.05).However,the incidence of complications of queasy,emesis and hunger feelings in the experimental group was lower than that of the control group,all with statistically significant differences(P<0.05).The scores of self-rating depression scale(SDS),self-rating anxiety scale(SAS)and visual analogue scale(VAS)of patients in the experimental group were all lower than those in the control group,and the scores of each dimension of general comfort questionnaire(GCQ)were all higher than those in the control group,all with statistically significant differences(P<0.01).The nursing satisfaction of patients in the experimental group was superior to those in the control group,with statistically significant differences(Z=22.571,P<0.001)).Conclusion The clinical application of ERCP in the perioperative ERAS-CNP treatment of calculus of common bile duct has a remarkable efficacy,which can promote postoperative recovery of patients,improve comfort,reduce complications and increase nursing satisfaction.
