Analysis of screening results of upper gastrointestinal cancer in Wenshang County of Shandong Province from 2007 to 2021
Objective To analyze the screening results of upper gastrointestinal cancer in the early diagnosis and treatment project on cancer of Wenshang County from 2007 to 2021.Methods In Wenshang County,the population aged 40 to 69 years old in townships or villages with high morbidity and mortality was selected as the target population by cluster random sampling method.The high-risk groups were selected by questionnaire survey of high-risk factors,and then endoscopic examination and pathological diagnosis were made,and the indices such as positive rate,early diagnosis rate and treatment rate were calculated.Results From 2007 to 2021,27,784 people underwent endoscopic examination,and 19,173 people underwent biopsy and pathological examination.353 cases of upper gastrointestinal cancer were detected,the detection rate was 1.27%,in which 272 cases were early cancer cases,and the early diagnosis rate was 77.05%.According to the annual analysis,there was statistically significant difference in the change of biopsy rate between 2007 to 2014 and 2015 to 2021(x2=202.27,P<0.05).There was statistically significant difference in the change of detection rate between 2007-2014 and 2015 to 2021(x2=34.44,P<0.05).Especially in the 2018 to 2021,the positive rate of screening remained above 2.10%,exceeding 2.00%as the target detection rate set by the national project team.There was statistically significant difference in the treatment rate between 2007 to 2014 and 2015 to 2021(P<0.05).There was no statistically significant difference in early diagnosis rate between 2007 to 2014 and 2015 to 2021(P>0.05).Conclusion The screening method of"upper gastrointestinal endoscopy plus pathological examination"can effectively detect upper gastrointestinal cancer and precancerous lesions after the high-risk population is evaluated through questionnaire survey,which is of practical significance to improve the cancer survival rate in the region.
