Evaluation of clinical efficacy of Fu's subcutaneous needling therapy on cervical spondylosis based on the volume of the posterior extensor muscle and sagittal parameters of the cervical spine
Objective To evaluate the efficacy of Fu's subcutaneous needling therapy on cervical spondylosis based on the volume of the posterior extensor muscle and sagittal parameters of the cervical spine,and its correlation with efficacy.Methods A total of 50 patients with cervical spondylosis admitted from January to August 2023 were selected retrospectively.All patients were treated with Fu's subcutaneous needling therapy and underwent cervical magnetic resonance imaging(MRI)and lateral X-ray examination before and after treatment.The picture archiving and communication system(PACS)was used to measure the sequence of sagittal parameters of the cervical spine on lateral radiographs.Image J software was used to measure the superficial extensor area(SEA)and deep extensor area(DEA)of C 3-7 upper endplate on MRI axial images.The ratio of DEA to corresponding cervical body area(CBA)was measured and analyzed.The sagittal parameters of the cervical spine and the volume of the posterior extensor muscle were compared before and after treatment.Pearson correlation analysis was used to analyze the relationship between volume of the posterior extensor muscle and sagittal parameters of the cervical spine,cervical function(JOA)score and curative effect.Results Compared to before treatment,there was a significant difference in the changes in sagittal parameters of the cervical spine after treatment.C 2-7 SVA and C 7 S were significantly reduced,while C 0-2 Cobb angle and C 2-7 Cobb angle were significantly increased.Compared to before treatment,the DEA/CBA ratio of patients after treatment significantly increased,with the most significant differences being observed in C 3,C 4,and C 5 indicators;The DEA/CBA ratio at the C 3 level was weakly negatively correlated with C 2-7 SVA(r=-0.380)and C 7 S(r=-0.322),while the DEA/CBA ratio at the C 4 level was moderately negatively correlated with C 2-7 SVA and C 7 S(r=-0.489,r=-0.477).There was no or extremely weak correlation between SEA/CBA ratio and sagittal parameters of the cervical spine.There was a strong or moderate correlation(P<0.05)between the volume of the posterior extensor muscle and the sagittal parameters of the cervical spine,as well as the JOA score and efficacy.Conclusion The effect of Fu's subcutaneous needling therapy on cervical spondylosis is significant,and there is a strong or moderate correlation between the volume of the cervical extensor muscle and the sagittal parameters of the cervical spine,JOA score,and therapeutic effect.
