Study on the diagnostic efficacy of tubercle bacillus culture combined with mycobacterium tuberculosis-DNA detection in bacterial negative pulmonary tuberculosis
Objective To explore the diagnostic efficacy of tubercle bacillus culture combined with mycobacterium tuberculosis-DNA detection in bacterial negative pulmonary tuberculosis.Methods A total of 188 patients with suspected bacterial negative pulmonary tuberculosis admitted to Yangjiang Public Health Hospital from September 2021 to September 2022 were selected for tubercle bacillus culture and mycobacterium tuberculosis-DNA detection.Tissue culture and pathology as the"gold standard".The diagnostic results and efficacy of tubercle bacillus culture,mycobacterium tuberculosis-DNA detection alone,and combined detection of bacterial negative pulmonary tuberculosis were compared.Results Out of 188 patients with suspected bacterial negative pulmonary tuberculosis,132 were confirmed positive,and tubercle bacillus culture test detected 115 positive and 73 negative cases.107 cases were positive and 81 cases were negative in the mycobacterium tuberculosis DNA-detection.Combined testing detected 134 positive cases and 54 negative cases.The sensitivity,negative predictive value,and accuracy(91.67%,79.63%,87.23%)of tubercle bacillus culture combined with mycobacterium tuberculosis DNA-detection for bacterial negative pulmonary tuberculosis were higher than those of tubercle bacillus culture test(78.79%,61.64%,79.26%)and mycobacterium tuberculosis DNA-detection(75.00%,59.26%,78.19%),with statistical significance(P<0.05).The specificity and positive predictive value(76.79%,90.30%)of tubercle bacillus culture combined with mycobacterium tuberculosis DNA-detection for bacterial negative pulmonary tuberculosis were compared with those of tubercle bacillus culture test(80.36%,90.43%)and mycobacterium tuberculosis DNA-detection(85.71%,92.52%),and there was no statistically significant difference(P>0.05).Conclusion The combination of tubercle bacillus culture and mycobacterium tuberculosis DNA-detection for the diagnosis of bacterial negative pulmonary tuberculosis is beneficial for improving diagnostic sensitivity,negative predictive value,accuracy,and high diagnostic efficiency.
