Meta-analysis of the therapeutic efficacy of the pararectus approach in the treatment of pelvic and acetabular fractures
Objective To systematically evaluate the efficacy of the pararectus approach in the treatment of pelvic and acetabular fractures.Methods The CNKI,Wanfang,VIP,PubMed,EMBASE,Cochrane Library were searched for literatures on the curative effect of pararectus approach and modified Stoppa approach in the treatment of pelvic and acetabular fractures.The retrieval period was from January 1,2014 to December 31,2023.The data of fracture reduction quality(Matta score),Harris score,modified Merle D'Aubigne-Poste L hip joint evaluation system and the number of complications were extracted,and the meta-analysis was carried out by using RevMan 5.1 software.Results The fracture reduction quality(Matta score)of the experimental group was higher,with OR=1.62,95%CI(1.11,2.35),P=0.01.The experimental group had fewer complications,with OR=0.57,95%CI(0.43,0.75),P<0.0001.The hip joint functions of the two groups were equivalent,with Harris score OR=1.36,95%CI(0.80,2.30),and P=0.26.The modified Merle D'Aubigne and Postel L scoring system(counting)had an OR of 1.24,95%CI(0.46,3.36),and P=0.67.The modified Merle D'Aubigne and Postel L scoring system(measurement)had an MD of 0.07,95%CI(-0.26,0.40),and P=0.70.Conclusion Compared with the modified Stoppa approach,the pararectus approach in the treatment of pelvic and acetabular fractures showed fewer complications,higher reduction quality but little difference,and no difference in hip function.
