Analysis of the reasons for prescription dispensing errors in outpatient pharmacies of county-level comprehensive hospitals and exploration of optimization strategies
Objective To analyze the reasons for prescription dispensing errors in outpatient pharmacy of the People's Hospital of Yizheng and summarize.Discuss relevant improvement measures based on the above situation,in order to reduce the overall dispensing error rate of outpatient pharmacies and improve the quality of pharmaceutical comprehensive services.Methods The total prescriptions and wrong prescriptions of outpatient pharmacy in the People's Hospital of Yizheng from 2021 to 2023 were collected and the error rate was calculated.The causes of errors were analyzed and the relevant improvement measures were elaborated.Results The total number of prescriptions in 2021 was 253,605,with an error rate of 0.016 ‰.In 2022,the total number of prescriptions was 320 418,with an error rate of 0.009 ‰.In 2023,the total number of prescriptions was 364 400,with an error rate of 0.127 ‰.The main reasons for errors included human factors,environmental factors,management factors,hardware equipment,and other aspects.Conclusion Prescription dispensing errors often occur,and ultimately are mainly affected by factors such as human factors,environment,management,and equipment.In the follow-up work of outpatient pharmacies,efforts should be made to strengthen personnel management and training,regularly maintain drugs and inspect equipment,improve the service awareness and sense of responsibility of staff,and ensure the rational and safe use of medication by patients.
