目的 探讨2型糖尿病患者发生视网膜病变相关实验室生化指标、相关影像学表现，分析其发生、进展的相关危险因素，为本病在临床的诊疗、预测提供一定的依据。方法 选取2021年11月至2022年8月芜湖市中医医院内分泌科收治的114例2型糖尿病患者为研究对象，按照是否有糖尿病视网膜病变分为观察组及对照组，收集两组一般资料、相关实验室生化指标及相关影像学表现，通过单因素分析及多因素logistic回归分析2型糖尿病患者视网膜病变发生、进展的相关影响因素。结果 收缩压(SBP)(β=-1。05，OR=0。35，P＜0。05)，空腹血糖(FPG)(β=-0。50，OR=0。61，P＜0。05)，餐后 2 h 血糖(2 hPG)(β=-0。66，OR=0。52，P＜0。01)，尿酸(UA)(β=-0。01，OR=0。99，P＜0。05)，尿微量白蛋白/尿肌酐(MALB/Gr)(β=0。02，OR=1。02，P＜0。05)，凝血酶时间(TT)(β=1。12，OR=3。07，P＜0。05)，颈部血管病变(β=2。60，OR=13。41，P＜0。05)，丙氨酸转氨酶(ALT)(β=0。15，OR=1。17，P＜0。05)，下肢血管病变(β=-3。39，OR=0。03，P＜0。05)是2型糖尿病患者发生视网膜病变独立影响因素(P＜0。05)。结论 FPG、2hPG、SBP、UA、MALB/Cr、TT、ALT、颈部血管病变及下肢血管病变为2型糖尿病患者视网膜病变发生及进展的独立影响因素。
Analysis of influencing factors for the occurrence of retinopathy in patients with type 2 diabetes mellitus
Objective To explore the laboratory biochemical indicators and relevant imaging manifestations of retinopathy in patients with type 2 diabetes mellitus,and analyze the related risk factors for its occurrence and progress,so as to provide a certain basis for the clinical diagnosis,treatment,and prediction of this disease.Methods A total of 114 patients with type 2 diabetes mellitus who were admitted to the Department of Endocrinology,Wuhu Hospital of Traditional Chinese Medicine from November 2021 to August 2022 were selected as the experimental subjects.They were divided into the observation group and the control group according to whether there was diabetes retinopathy.The general data,relevant laboratory biochemical indicators and relevant imaging manifestations of the two groups were collected,and the relevant influencing factors of the occurrence and progress of retinopathy in patients with type 2 diabetes mellitus were analyzed by single factor analysis and multi factor logistic regression.Results Systolic blood pressure(SBP)(β=-1.05,OR=0.35,P＜0.05),fasting plasma glucose(FPG)(β=-0.50,OR=0.61,P＜0.05),2-hour postprandial glucose(2 hPG)(β=-0.66,OR=0.52,P＜0.01),uric acid(UA)(β=-0.01,OR=0.99,P＜0.05),microalbuminuria/urinary creatinine(MALB/Cr)(β=0.02,OR=1.02,P＜0.05),thrombin time(TT)(β=1.12,OR=3.07,P＜0.05),cervical vascular disease(β=2.60,OR=13.41,P＜0.05),alanine aminotransferase(ALT)(β=0.15,OR=1.17,P＜0.05),lower limb vascular disease(β=-3.39,OR=0.03,P＜0.05)is an independent influencing factor for retinopathy in type 2 diabetes patients(P＜0.05).Conclusion The FPG,2hPG,SBP,UA,MALB/Cr,TT,ALT,cervical vascular disease and lower limb vascular disease are independent influencing factors for the occurrence and progress of retinopathy in patients with type 2 diabetes mellitus.
