贲门癌手术患者1007例临床及诊疗状况分析

摘要
目的 分析1007例贲门癌患者的临床特点及诊疗状况,为临床确诊并选择最佳诊疗方案和合理利用医疗资源提供科学参考.方法 将2002年1月至2004年12月在河北医科大学第四医院手术治疗的1007例贲门癌患者的临床资料录入计算机,采用SPSS 10.0统计软件进行回顾性分析.结果 ①60.38%患者有下咽不适或不顺畅为首发症状;②病灶侵及深度至周围组织的占75.07%,有46.87%淋巴结转移;③有吸烟史、饮酒史患者分别为48.76%和31.18%;④有遗传家族史占22.24%;⑤仪器检查率:CT21.75%、食管造影41.41%、食管镜或胃镜55.11%;⑥60～69岁发病人数最多.结论 就诊贲门癌患者多数为中、晚期并有一定的老龄化趋势;贲门癌的发生与吸烟、饮酒相关且有一定的家族倾向;CT、食管造影、食管镜或胃镜检查尚有较大应用空间.
Abstract
Objective To provide scientific referrance to the rational use of medical resources as well as giving the best treatment for the patients by investigating the diagnosis and treatment of cardia cancer patients. Methods The clinical data of 1007 cases of cardia cancer admitted in the Fourth Hospital of Hebei Medical College from January 2002 to November 2004 were retrospectively analyzed by SPSS10.0 software. Results 1. There were 60. 38% patients with dysphagia as initial symptoms . 2. There were 73.07% patients with the carcinoma invading up to the soft surrounding tissue and 46.87% with the lymph node metastasis. 3.48.76% of the cases were smokers,and 31.18% of them were drinkers . 4. There were 22.24% with a positive family history of cardia cancer. 5. The per-cents of cases who had ever received CT,radiography,endoscopy examination were 21.75% ,41.41% and 55.11% respectively. 6. Most of the patients aged between 60 and 69. Conclusion The most of the patients are in the middle and terminal cancer with a tendency toward aging. Risks of the cardia cancer are positively associated with smoking, drinking,and family history of cardia cancer. There is good expanding space on the rate of the instrument's utilization.
